Clinical leadership and expertise are identified as essential for delivering the North West London Integrated Care System vision and objectives. The clinical leadership works together across the Integrated Care System to focus on service improvement, service redesign and the development of care pathways from primary care through to secondary care and community-based services.

This is an exciting opportunity to join a dynamic and committed Integrated Care System health and care leadership team to shape the impact of primary care leadership in North West London.

The Opportunity

The Medical Director for Primary Care has a critical role within the Integrated Care System. The post holder will lead the Borough Medical Directors in all eight place-based partnerships within North West London to deliver complex changes and integrated working across health and social care.

This role requires strong, strategic leadership skills, critical reasoning, and numeracy, with the experience of stewardship of significant public funds, good governance and delivering impactful public outcomes.

The ideal candidate will bring with them a track record of leading transformational change, the ability to establish a stakeholder-led culture across the system, build trusted partnerships which drive change and improvements in care, have great personal integrity and champion diversity, inclusion, and equality of opportunity for all.

NHS North West London Integrated Care Board (NHS NWL ICB) is a statutory body responsible for planning and allocating health and care resources to improve the lives of people in eight boroughs across North West London: Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, Royal Borough of Kensington and Chelsea and Westminster.

Job responsibilities

The Medical Director for Primary Care is a significant role in the NWL ICS clinical leadership structure. The role is distinct from the statutory role of Partner member for Primary Care for NHS North West London. The primary focus of the post will be to provide clinical leadership for primary care, taking leadership responsibility for improving all aspects of clinical care and outcomes.

You will support the delivery of the primary care and related aspects of the NHS Long Term Plan priorities in respect of improving health outcomes and primary care services and implementation of the Fuller Stocktake.

The specific elements of the portfolio will vary over time but may include specific responsibility for areas such as:

further developing primary care networks

improving the interface between primary and secondary care, pharmacy and prescribing issues

transforming how elective, urgent and emergency care interface with primary care

working towards new models of care, all within the context of the emerging integrated neighbourhood teams.

This is a key role in the ICS clinical structure and specific deliverables are:

Population health and healthcare

  • Delivering high standards: To provide clinical leadership and insight to support the delivery of high standards of care
  • Improving quality and outcomes
  • To support the improvement of outcomes for patients and drive up the quality of care by providing clear clinical leadership to adopt innovative approaches and align tools and levers,

To ensure primary care is positioned as a key participant in the development, commissioning and delivery of services within the integrated care partnership maximising use of appropriate levers, incentives and metrics to drive progress on improving quality.

To provide a primary care perspective to the design of patient pathways

To support the embedding of quality and outcomes in the disciplines relating to dentistry, optometry and community pharmacy services.

To champion the delivery of a clinical effectiveness approach for primary care throughout North West London.

Manage emerging quality and safety initiatives as they relate to primary care

Tackling unequal access, experience and outcomes

To uphold organisational policies and principles on the promotion of equality

To create an inclusive working environment where diversity is valued, everyone can contribute, and everyday action ensures we meet our duty to uphold and promote equality

To influence and shape the commissioning of services to eliminate social and cultural inequalities for disadvantaged groups

Champion the use of data and evidence on a standardised basis to drive health and inequalities improvements

Provide relevant and timely specialist advice and guidance and work with key stakeholders to investigate the causes of any variance from plan/delivery targets and contribute to the implementation of solutions

To encourage the development of good employment practice within the profile of primary care organisations across North West London

To support the aspirational goal of social and economic development and the creation of social value within North West London

To support delivery of the NHS England commitment towards the Net Zero target and implement a strategy towards sustainability

To support the implementation of the Fuller Stocktake within North West London as part of the delivery North West London strategy.

Streamlining access to care and advice

Develop a single system-wide approach to managing integrated urgent care to guarantee same-day care for patients and a more sustainable model for practices

Assist systems with integration of primary and urgent care access

Providing more proactive, personalised care with support from a multidisciplinary team of professionals

Support all PCNs to be a key component of the emerging integrated neighbourhood teams

Embed primary care workforce as an integral part of system thinking, planning and deliver

To support models of care that promote improved health outcomes for those with long-term conditions or mental health issues

Helping people to stay well for longer

Work alongside local people and communities to facilitate acting sooner to help those with preventable conditions

To promote the utilisation of digital technology to improve access, improve how services are delivered and enable better planning to meet the needs of local people and communities.

To coordinate a primary care forum with the expectation of developing a primary care collaborative.

Create a clear development plan to support the sustainability of primary care

To support primary care organisations in considering future models of delivery

To actively promote the strategy and performance of primary care within North West London with colleagues across neighbouring ICBs and the London and National Primary care team

To be an active member of the London Primary Care clinical senate

To participate in the London Primary Care Board and associated workstream

Functional Responsibilities:

Leadership

To provide leadership for Borough Medical Directors and NWL Primary Care Clinical Leads and together with the ICS Medical Director set work plans, objectives and conduct regular reviews of outcomes for Borough Medical Directors.

Within the primary care governance framework take on the role of Chair of the NWL Primary Care Executive group and to Chair the NWL Primary Care Programme Board

Co-Chair the NWL Clinical Advisory Group and the NWL Local Care Board.

Line Management

Line management responsibility for the Borough Medical Directors and a cohort of NWL clinical leads.

Analytical

Awareness of risks to the programme/portfolio and ensure that escalation processes are followed.

Provide coordination of and lead relevant meetings, reporting attendance and providing information advice and support where requested.

Ensure that data collected is analysed, reported as appropriate and monitor the processing of data and information.

Communications and Engagement

Support co-production as a way of working ensuring input from service users, clinical colleagues and ensuring there are clear and effective lines of communication at all levels with all stakeholders.

Provide clear communication and visibility concerning the strategic development of primary care services with the population of North West London using the principles of co-design and co-production.

Communicate information, risks, issues and dependencies, including briefings and reports to the programme team, sponsors and a range of internal and external staff.

Support the development of internal and external communications where required by regular contact with the teams, stakeholders and Communications team.

Support the relevant NHS NW London programmes in the preparation of correspondence and papers.

Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams.

General Financial and Physical Resources

Deliver against organisational objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.

Continually strive for delivering project/function outcomes, value for money and greater efficiency

Contribute to the financial delivery of the project ensuring it is cost effective and delivered on time.

Information Management

Work with key stakeholders to develop and implement local project data collection systems that will provide accurate and timely data.

Research and Development

Actively lead and contributes to the development of key performance indicators for the successful assessment of variation. Ensuring there are processes in place for spreading and sharing learning and outcomes.

Work with local academic organisations, drawing on their expertise and resource to support service development and evaluation.

Membership of the Research and Innovation sub-committee of the ICB.

Person Specification Qualifications
  • Full Registration with GMC without any conditions or restrictions on licence to practice
Experience
  • Recent experience of senior clinical leadership at service, divisional or borough level for relevant years
  • Proven track record of delivery of significant clinical change programmes at speciality of divisional or borough level
  • Experience of working with patient groups and wider external stakeholders
  • Demonstrable leadership skills and an ability to influence and motivate others.
  • Judgement: having the ability to understand and interpret clinical decisions for others and make informed judgements concerning clinical priorities.
  • Be innovative and responsive to change.
