Main duties of the job

The Head of Clinical Services willwork with the Partners as part of the Management Team with responsibility formanaging the range of health care professionals employed by the Practice. Thepost holder will also work autonomously within their professional scope ofpractice providing care for patients.

The post holder will work with theExecutive Manager developing the strategic direction of the Practice, and inparticular ensuring our clinical teams are working efficiently and effectivelyto deliver the outcomes desired for our patients.

The post holder will directly manageClinical Team Leaders and will support them in managing their teams.

Thepost holder will work with the Financial Performance Manager to ensure we meetthe national and local targets.

Job responsibilities

MajorDuties

Managerial

To provide clinicalleadership for all health care professionals in the Practice other thanGeneral Practitioners.

To develop andimplement plans to use the diverse workforce to best meet the needs of ourpatients.

To work with the non-clinical management toensure local and national targets are met.

To ensure all clinical staff receive thetraining and development required to fulfil their roles.

To support the Executive Manager in ensuring allclinical services meet CQC standards.

With the support of Partners to provide clinicalsupervision to a diverse range of health care professionals.

Oversee therecruitment, training, and supervision of all Clinical Team members.

Provide continuedprofessional development opportunities for team members.

Conduct regularperformance reviews, providing constructive feedback to achieve positiveoutcomes.

Clinical Practice

To carry out a fullhealth assessment of patient health care needs. To provide advice, initiatetreatment, and refer to another member of the team as appropriate.

To provide and maintain high standards.

Prescribe products to meet the needs of patientswithin the practice population.

Refer to other primary, community and secondarycare members within locally developed pathways.

To work in partnership with GeneralPractitioners and other professionals to develop protocols, standards andaudit tools to facilitate effective delivery of care.

Home visits as appropriate to patient careincluding Care Homes.

Work with others to change policies to improvehealth and well-being and reduce inequalities.

Therapeutic Monitoring

Checking compliance with and adherence to appropriatetreatments using a holistic patient-centred approach

Recognising abnormalities.

Identifying the impact oftreatment and implementing or recommending changes as appropriate.

Recognising issuesand gathering sufficient information to refer (e.g., drugs; domesticviolence; child protection; vulnerable adults; senior patients; socialproblems)

Education, Training and Professional Development:

Maintain continued educationby attendance of courses and study days as deemed useful or necessary forprofessional development and develop and maintain a personal learning plan inline with revalidation requirements.

Contribute to the education ofprimary and community care clinicians of all disciplines with specialresponsibility for nursing staff.

Create a learning environmentthat promotes the concept of life-long learning.

Contribute and participate asrequired in any research projects within the Practice.

RoleResponsibilities

Managerial:

Provide clinical leadership for the Practice

Develop and sustain a culture of team working across the Practice.

Be innovative in developing new ways of working where appropriate andsupport staff through change.

Manage the team performance including regular appraisals.

Work with the Executive Manager and wider team to ensure services areprovided efficiently and effectively and integrate with the wider business ofthe practice.

Clinical

Undertakeconsultation at an advanced level, using expert knowledge and clinical skillsto ensure patients receive high quality care, delivered in a timely manner.

Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly.

Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complexneeds.

Proactively identify, diagnoseand manage treatment plans for patients at risk of developing a long-termcondition (as appropriate).

Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatmentmethods into a management plan.

Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice.

Makedirect referrals to secondary care, mental health and social services withinlocally agreed pathways.

Maintainaccurate and contemporaneous records using the Practice clinic system.

Protectand promote the health and well-being of patients, their relatives andcarers.

Workcollaboratively with stakeholders across the Health and Social Care sectors,and in partnership with patients and/or carers, develop effective, efficientand preventative care plans.

Mentorand support other clinicians and undertake clinical peer reviews.

Liaisewith the Practices rota GP and other GPs as required.

ProfessionalRole

Maintain an up todate professional registration

Monitor theeffectiveness of own clinical practice through the quality assurancestrategies such as the use of audit and peer review.

Maintain ownclinical/professional expertise by participating in clinical activities,personal development planning, clinical governance and revalidation.

Participate incontinuing professional development opportunities to ensure that up-to-dateevidence-based knowledge and competence in all aspects of the role ismaintained.

Training requirementswill be monitored by yearly appraisal and will be in accordance with practicerequirements.

Keep up to date withpertinent health-related policy and work with the practice team to considerthe impact and strategies for implementation.

Pro-actively promoteyour role within the Practice.

Promote aprofessional approach at all times by dress and attitude to patients andcolleagues.

It will be necessaryto attend and contribute to various practice meetings as requested. (The onlyreason for not attending will be annual, study or sick leave).

Support thedevelopment of pathways with tertiary services to enable the Urgent CareClinics to become non-GP led.
