In this role, you will provide comprehensive mental health care and support to individuals within the community, ensuring they receive the highest standard of care. Key Responsibilities:

  • Conduct thorough mental health assessments and develop individualized care plans.
  • Provide therapeutic interventions and ongoing support to service users and their families.
  • Collaborate with multidisciplinary teams to ensure coordinated care.
  • Monitor and review care plans, adjusting as necessary to meet the evolving needs of service users.
  • Maintain accurate and timely clinical records in accordance with NHS guidelines.
  • Participate in case conferences, clinical supervision, and training sessions.
Requirements:
  • Registered Mental Health Nurse (RMN) with current NMC registration.
  • Minimum of 2 years’ experience as a Band 5/6 CPN or in a similar role.
  • Strong assessment and clinical decision-making skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of a multidisciplinary team.
  • Experience in community mental health settings is highly desirable.
  • Full UK driving license and access to a vehicle.
