Job summary

There is an exciting opportunity for an experienced and enthusiastic General Practice Nurse to join our Practice and work with our friendly and supportive team.

Main duties of the job

As a Practice Nurse at Newton Place Surgery, the successful candidate will work as a member of our multidisciplinary team ensuring that our nursing services are delivered safely and effectively to our patients.

Duties may include:

Cytology Family planning Childhood & adult immunisations All Treatment Room duties Management of patients with chronic diseases Anticoagulation monitoring (advantage) Travel health and vaccinations (advantage) Diabetes reviews Asthma/ COPD reviews (advantage)

The role may also require you to work enhanced access hours during the week and some Saturday mornings.

Interviews will take on place 22nd and 23rd July.

About us

Newton Place Surgery has a large multidisciplinary team including 17 GPs, practice nurses, Associate Practitioners and HCA. We also employ other allied health professionals such as a Mental Health Practitioner, First Contact Practitioners, ANPs, Paramedic Practitioners, Clinical Pharmacists and GP Assistants. We are a forward-thinking practice, committed to delivering high-quality care to our 19,400 registered patients.

Newton Place Surgery is a vibrant purpose-built surgery with a great family feel, set in the beautiful market village of Faversham in the countryside of Kent. The practice has good transportation links, being less than 1 mile from Faversham train station and close to the M2 turn-off.

We offer a variety of additional on-site services such as ultrasound, audiology, physiotherapy, women's health, minor surgery, ECGs, and spirometry and we are also a covid vaccination site. The practice is actively involved in research and employs a small team to support this. We are also a training practice that hosts ST2, ST3, medical students, PAs, paramedics, and student nurses.

Job responsibilities

To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control,health promotion, chronic disease management, health promotion, well women clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Newton Place Surgery must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager Undertake cervical screening To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice To develop, implement and embed health promotion and wellbeing programmes To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF As required, to diagnose and manage acute and chronic conditions To prescribe medication for patients in accordance with PGDs and own scope of practice To review patient medications to enhance compliance To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages To co-ordinate the provision of travel medicine services To request pathology services and process pathology results as required To maintain accurate clinical records in conjunction with extant legislation To participate in contraceptive clinics To ensure compliance with the practice chaperone policy To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To implement vaccination programmes for adults and children To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) Person Specification
