Job summary

Care Coordinators play an important role within a PCN (Primary Care Network) to proactively work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. Care Coordinators will be based within our main hub at Goldsmiths Community Center and the option of occasionally working from one of our practices locations. We provide a high quality service to all our patients and practices and believe in exceptional patient care. There will be innovative and exciting services launching which will change the way we provide services to our patients.

Main duties of the job

The CareCo-ordinator is the link between the patient and the NHS. They workclosely with our patients to help them improve access, health outcomes andengagement with their health providers.

Sevenfields PCNhas higher than average levels of deprivation and BAME populations; patientshave significantly poorer uptake of child immunisations when comparing withnational targets. Practices have a high prevalence of diabetic patients;NHS Healthchecks are important to highlight early intervention for pre diabeticpatients.

Sevenfields PCN iscommitted to tackling health inequality and our care coordinators are a vitalpart of this work plan.

About us

Sevenfields PCN is embedded into its community with active link workers and a staff base at Goldsmiths Community Centre which also offers vaccinations and other services. We have a real passion for the health and wellbeing of our residents and have invested in four outdoor gyms for our local communities using surpluses generated by our vaccination work which was largely delivered by our active volunteer team.

Our local population is somewhat older than other parts of Lewisham and contains several large care homes. It includes areas of extreme socioeconomic deprivation and the PCN is keen to find innovative ways of addressing the health challenges that accompany this.

All of the GP practices in Sevenfields are shareholders (members) of our PCN company which has a Board that provides clear leadership through strong GP practice representation.

As is the case elsewhere, Primary Care in Lewisham is under pressure. The challenge for Sevenfields PCN is to work in partnership with our local practices, GP Federation and Integrated Care Board (at South East London and Place levels) across our neighbourhood to help shape and implement the transformation of Care in our locality ensuring that it can both survive and thrive while still contributing to the delivery of new models of care that are self-sustaining financially.

Job responsibilities

Assist patients in navigating the NHS services and productsincluding showing patients how to use Apps and websites to book appointments,request for medication or access information about health or health services.

Help in the planning and arranging of patient care and providingsupport to patients such as support filling in applications,reading/understanding literature.

Supporting GP practices by conducting basic health services suchas flu/covid-19 vaccinations and health checks

Conduct patient assessments and where required, signpostpatients to the appropriate service

Supporting the implementation of PCN, local and nationalpolicies such as those aimed at reducing health inequalities

Qualifications

Essential

Minimum of 5 GCSEs/O levels Evidence of ongoing learning Demonstrate commitment to professional and personal development

Experience

Experience of working in a team Ability to work against deadlines and prioritise workload Excellent communication skills, both verbal and written.

Desirable

Minimum of 2 years administration experience in any setting Experience of General Practice Experience and knowledge of living or working in Lewisham
