Jewish Care is seeking a dedicated and highly organised Clinical Care Coordinator to support the Registered Manager and Care Managers at our Mental Health Residential Care Homes. This role is pivotal in ensuring the seamless operation of Sidney Corob House, which supports adults over 50 with mental health needs, and Jack Gardner House, a safe and friendly environment for Jewish adults with mental health needs (aged 18+).

The Care Coordinator for Mental Health Care Homes will play a crucial role in operationalizing two mental health care homes. This position involves coordinating clinical and care audits, ensuring compliance with regulations, and enhancing the quality of care provided to residents. The ideal candidate will possess strong organisational skills, a deep understanding of mental health care, and the ability to work collaboratively within a multidisciplinary team.

This post is working Monday to Friday for 39 Hours per week

What you will be doing:

Support to Registered Manager and Care Management Team:

  • Assist the Registered Manager in the day-to-day operational management of the care homes.
  • Provide administrative and logistical support to ensure the smooth running of both facilities.
  • Participate in strategic planning and implementation of care home policies and procedures.

Resident Care Coordination:

  • Conduct comprehensive clinical assessments of residents’ mental health needs.
  • Coordinate and monitor the delivery of clinical and care services to ensure residents receive high-quality care.

Team Collaboration and Communication:

  • Facilitate effective communication among the care management team, care staff, residents, and families.
  • Support the Registered Manager and Care Managers in organizing and leading regular clinical reviews and meetings relevant to resident progress and addressing any concerns.
  • Act as a liaison between care homes, healthcare providers, and community resources to support residents’ needs.

Compliance and Quality Assurance:

  • Ensure all care activities comply with relevant regulations, policies, and standards of practice.
  • Maintain accurate and timely documentation, including assessments, audits, and progress notes.
  • Participate in internal audits and contribute to quality improvement initiatives.

Staff Training and Development:

  • Support the training and development of care home staff on mental health care practices, care coordination, and effective communication.
  • Promote a culture of continuous learning and professional development within the care team.

Resident Advocacy and Support:

  • Advocate for residents’ rights and well-being within the care homes and the wider community.
  • Provide emotional support and guidance to residents and their families.
  • Assist residents in accessing healthcare services.
What you will need:
  • NVQ or QCF level 3/4/5 (Essential)
  • Mental health – RMN or equivalent (Essential)
  • Previous experience working with older people and leading a team in a care or nursing home environment (Essential)
  • Proven experience in the day-to-day management of a care or nursing home environment (Essential)
  • Experience of leading teams in a supervisory role (Essential)
  • Experience of security and building management (Essential)
  • Experience of the administration of medicine (Essential)
  • Experience of dealing with emergency care (Essential)
  • Experience of working with people living with dementia (Essential)
  • Excellent understanding of social care and its regulatory system (Essential)
  • Knowledge of all aspects of care for older people including dementia and end-of-life care and issues of capacity (Essential)
  • Knowledge, understanding, and commitment to Safeguarding and DOLS (Essential)
  • Working knowledge and strong commitment to person-centred care and the rights of older people and those with physical, sensory, and cognitive impairments (Essential)
