Job summary
As a Healthcare Support Worker, you will be responsible for maintaining the safety and wellbeing of our patients, deliver personal care whilst promoting privacy and dignity and support patients with their nutritional, skin, continence and mobility care. We need our HCSWs to be flexible, working a variety of shifts. Our rosters are provided 6 weeks in advance with the possibility of a bank contract too.
Snowdon ward is a 14 bed neurorehabilitation unit caring for patients with complex physical and cognitive challenges, with the aim of enabling independence and quality of life by supporting inpatients with their personal care and personalised rehabilitation goals. We also currently operate another level 3 rehabilitation ward and a Neuropsychiatric unit which provide a range of specialist rehabilitation. The post comes at an exciting time for rehabilitation within Southampton, South-West Hampshire and nationally, with our new specialist rehabilitation unit - South of England Rehab Centre launching in October 2024.
You will act as a role model for team members, supporting new or less experienced colleagues.
This role will assist and support qualified staff and carry out assigned tasks as directed, without supervision. You will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.
Main duties of the job
Actively promote recovery-focussed care with patients, in conjunction with other health professionals, including achievement of set goals.
Support patients with their rehabilitation and recovery, respecting them as individuals, supporting them and understanding their right to express their needs.
Be the main provider of personal care for patients where required.
Facilitate and lead patient-focus groups and activities.
Use appropriate approved conflict resolution and management of aggression techniques ( PMVA), to de-escalate challenging situations as required for the role.
Accurately assess, describe and record observations on progress or change in both patients' mental and physical state using appropriate assessment tools, escalating any concerns as appropriate.
Undertake clinical and environmental risk assessments and document key risk areas, seeking guidance from more senior staff when dealing with cases indicating a high level of risk.
To raise incidents appropriately via the reporting system in an accurate, objective and concise manner.
Provide information on improving health and wellbeing and signposting to other agencies as appropriate.
To undertake escort duties to other hospitals or care settings as required.
To ensure timely and accurate recording of patient information on patient systems.
About us
We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
