Job summary

Weare looking for an experienced Practice Nurse to work between us and other practiceswithin our Primary Care Network (PCN), focusing primarily (but not exclusively)on the more advanced aspects of Practice Nursing such as Chronic DiseaseManagement blood pressure and diabetes.

Main duties of the job

We arelooking for a colleague with excellent interpersonal and clinical skills withexperience working in a GP Practice. Competence in chronic disease managementis essential (particularly asthma, COPD, diabetes and hypertension) along withstandard nursing duties such as immunisations, cervical screening, dressings,phlebotomy etc.

Thesuccessful applicant would also have the opportunity to take on the EducationalNurse Lead for Mewstone PCN (a role which would include attending a monthly ICBnurse lead forum and lead the quarterly PCN nurse forum). There would also bethe opportunity to develop a mentoring and / or training role with qualifiednurses new to Practice Nursing and / or Student Nurses. This would be anoptional element.

We areable to offer flexibility with regards to hours and there is also the option forperiods of extended leave during school holidays. Remuneration would reflectskill level and exact job specification.
