Job summary

TheSandwell Community Dementia Support Service (SCDSS) is commissioned by SandwellMetropolitan Council and the Black Country and West Birmingham ClinicalCommissioning Group. As lead provider, we are working alongside seven otherlocal organisations; Agewell CIC, Dementia Pathfinders CIC, West BromwichAfrican Caribbean Resource Centre, St Albans Community Association, BUDS, AgeUK Sandwell and Alzheimers Society to provide this service across SandwellMetropolitan Borough.

Main duties of the job

As a Community Dementia Adviser, you will be required toprovide relevant information in a variety of formats to meet individual needsas well as signposting and/or referring them to appropriate services as well asencouraging engagement in community activities.

Job responsibilities

Following the agreed service pathway, the Community DementiaAdviser will:

Work with individuals living withdementia and their family carers, providing maintenance support and call careaccording to the service casework model.

Work collaboratively with DementiaNavigators to facilitate interventions for people with dementia and theirfamilies requiring intensive support during periods of transition, complexityand crisis, involving statutory sector services as appropriate.

Recruit, deploy and support volunteers,to encourage and facilitate people with dementia and family carers toparticipate in group activities; and to offer one-to-one befriending forindividuals experiencing dementia, helping to provide respite for carers.

Lead initiatives to create dementiafriendly communities in the locality where they are based.

Person Specification

Qualifications

Essential

Level 2 dementia qualification (or willingness to work towards achieving this qualification) or a similar qualification and/or equivalent experience, commensurate with the responsibilities of this role.

Experience

Essential

Knowledge of dementia, person-centred approaches, issues that affect family carers and availability of local services. Experience of working in a busy environment and supporting others in a team. Record-keeping skills and writing ability of a high standard suitable for written communication with service users and professionals, production of publicity materials and reports. Excellent IT skills with confident use of MS Office packages including Word, Excel, Outlook and Power Point. An understanding of client confidentiality (GDPR). Full current UK driving licence and access to a vehicle to travel independently across the service area. Knowledge of and ability to work to Adult Safeguarding Procedures, Lone Working Procedures and the Mental Capacity Act. Please see attached Job Description and Person Specification for full details.
