As part of ongoing developments, we are redesigning unplanned mental health services in Doncaster and have an exciting opportunity for a based Home Treatment Practitioner to utilise their clinical skills and experience in a multi-disciplinary home treatment team working with varied and complex mental health presentations.

The role is to work as part of a team assessing the risks and needs of service users to plan and evaluate their care using a collaborative approach with service users and their carers. You will work alongside other members of the multi-disciplinary team to ensure effective care delivery, including the use of evidence-based interventions, risk assessment and management and safe discharge processes.

The team works in partnership with third sector and peer organisations to deliver person centred care, therefore transparency and openness is vital.

The successful applicant will have a passion and desire to work intensively with individuals who are acutely unwell during a relapse of their mental health, within the community. You will also work alongside their family, carers and wider care teams. A recovery and strength-based focus and passion for delivering evidence-based treatment is therefore essential.

Main duties of the job

To work as a member of a Multi-Disciplinary Team reporting back to colleagues, both verbally and in writing and seeking advice, guidance and support where necessary.

To provide each patient for whom they are a Lead Professional with an agreed care plan, containing review dates, in accordance with Community Mental Health Nurse and Care Programme Approach standards.

To actively seek to contribute to setting standards of care, to generate and implement quality initiatives.

To promote compliance with medication and other treatments prescribed, noting and reporting non-compliance and side effects as appropriate.

To develop and maintain healthy partnerships with all stakeholders including statutory, third sector and voluntary agencies.

To advise patients, carers and significant others of the available resources. Referring assessed need to those agencies best able to meet these in the community and facilitate use of these recourses.

To understand the requirements of the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Care Programme Approach, Deprivation of Liberty safeguards as it applies to a psychiatric liaison service.

To carry out the duties of a Lead Professional as appropriate following the principles of CPA.

Qualifications

Essential

RMN or equivalent Demonstrable experience and clinical expertise in managing individuals who are acutely unwell. Ability to risk assess and co- produce risk management and contingency planning Knowledge of working with trauma and delivering Trauma informed interventions. Experience of working work across organisational boundaries and being able to develop and maintain multi-professional/agency partnerships.

Desirable

Experience in managing individuals with substance misuse. Psychosocial interventions Experience of working work across organisational boundaries and being able to develop and maintain multi-professional/agency partnerships. Self-harm intervention and harm minimisation skills
