Salary: £ - £/hr depending on experience

PLUS, a Golden Hello of £1500 paid intwo instalments over 6 months (External applicant only)

Hours: 23 hours per week on a rolling rotabasis (2 long days per week)

Main duties of the job

It will be the responsibility of the post holder to, exerciseclinical expertise, levels of judgement, discretion and decision making inclinical care, as demonstrated under agreed parameters within the 4 pillars ofadvanced practice. To provide a high standard of care for patients presentingwith undifferentiated and undiagnosed minor injury needs. Please note, we see injury only at this service and thosepatients who present with illness are offered a triage service.

Job description

Job responsibilities

The post holder will:

Work with and champion the unit in conjunction with Senior Managerialcolleagues

Support the Management team and Clinical manager to ensure all serviceKPIs/Targets all are met and looking for improvement over and above the setcommissioned targets.

Ensure mandatory training is kept up to date and receive suitabletraining/CPD for their ongoing development needs.

Practice autonomously, as an independent practitioner and non-medicalprescriber, participating in the care of and undertaking assessment, diagnosis,treatment / referral and discharge of patients presenting with minor injury.

Identify the need for and initiate and interpretrelevant diagnostic investigations and results (X-Ray).

To safely and competently undertake invasive andnon-invasive procedures to establish a diagnosis.

Be able to independently undertake telephonetriage.

To work within the parameters of currentnon-medical prescribing legislation

Monitor and lead improvements to standards of care through supervisionof practice, clinical audit, and evidence-based practice, teaching andsupporting professional colleagues and the provision of skilled professionalleadership.

Contribute to the Clinical Governance agenda within the service byleading audit and research as part of the multi-disciplinary team within thearea of expertise.

Act in accordance with ones own Governingprofessional body

Provide professional and clinical advice within themulti-disciplinary team.

There may be occasions where Practitioners areasked to work across various sites in our Lancashire team within FCMS.

Knowledge, skills and experience required:

Relevant professional qualification

Experience of working independently as an unsupervised practitioner inthe primary or urgent care setting.

Experienced in undertaking telephone triage and remote prescribing.

Teaching and mentorship

Key results areas

Clinical

The post holder will:

Assess, treat, and advise patients presenting with minor injury needs.

Lead the shift within a small team

Provide supervision of the clinical team, assisting and developing theirteam and individuals

When required ensure patients are referred on to other members of theprimary health care team or secondary care as necessary and support and guideothers as required.

Ensure all practice is safe and effective and remains within boundariesand scope of competence of individuals and to acknowledge limitations.

Work at an advanced level of practice reflected under the 4 pillars ofadvanced practice of the HEE.

Perform investigatory procedures, including assessing patients withminor injuries, request, and review x-rays, treat minor injuries /implementexpanded roles in practice.

In general, the post-holder will be expected to undertake all the dutiesand responsibilities associated with an expert practitioner working withinUnscheduled Primary Care and out of hours.

Develop and maintain clinical policies, procedures, and protocols.

Ensure any patient information is up-to-date and produced in appropriateformats.

Ensure appropriate use of resources to meet service needs.

Recognise and act as an advocate for patients, carers, service, and theorganisation.

Carry out annual review of best practice/national standards andimplement recommended changes.

Promote workforce integrity (empower professional accountability withinthe clinical team, ensure accreditation process followed, complaintsmanagement, competency assessment, ensuring mandatory training requirementsfulfilled, robust clinical induction)

Maintain a sound knowledge of NHS policy and NICE guidance in relationto scheduled and unscheduled care.

Education and training

The post holder will:

Identify own development needs in line with service requirements withina personal development plan.

Develop and distribute educational leaflets in relevant formats, forpatients relating to services and patient pathways.

Ensure personal, peer support and clinical supervision needs are met.

Research and development

The post holder will:

Evaluate clinical practise through audit of service and develop andmanage strategies to address any shortfalls.

Undertake audits as required by the service working with the clinicalmanager.

Demonstrate knowledge of current research in all aspects of their workand advise others on the implementation of relevant research findings.

Initiate and/or become involved in research projects in response toidentified needs.

Disseminate results/research/ audit findings via local teams and/ornational/international publications.

Implement research findings appropriate to practice.

Promote the shared objectives of the multidisciplinary team by workingclosely to ensure that best practice is achieved utilising both ClinicalBenchmarking and Essence of Care processes.

Most challenging part of the role.

Ability to work under pressure in an ever changing, challengingspecialist environment whilst maintaining excellent standards of patient care.

Registered Practitioner Advanced Practice qualification/ RCN Credentialled practitioner with experience dealing with both children and adults / HEE portfolio. Substantial clinical experience within an unscheduled care setting, working at an Advanced level Non- medical prescribing qualification
