An excitingopportunity has arisen for an experienced registered nurse to join theDischarge to Assess team.

Discharge to Assess (DTA) is a dynamic and multi-skilled teamconsisting of nurses, social workers, occupational therapists,physiotherapists, community care workers and support workers. Our purpose is tosupport a safe and timely hospital discharge of people who are medically fit,and no longer need hospital care, to a place more suitable to the personsneeds. Assessments and care provision can then be tailored to support people toregain strengths and skills so that they can live as independently as possible.

Thisrole requires specialist nursing expertise to undertake and be responsible forcompletion of timely and comprehensive holistic evidence-based assessments, ina range of settings, and to apply the relevant nationally approved criteria toestablish eligibility for services and care.

Main duties of the job

The post holder will be clinically responsible for theprovision of quality nursing for the Discharge to Assess (DTA) Team whoare referred from the acute general hospital setting. Those referred may experience a wide variety of healthconditions and social care situations.

To work as a member of a Multi-professional Team, deliveringassessments and intermediate care ensuring high standards of clinical practiceare set, monitored and a worker in DTAyou will use broad ranging generic skills and capabilities while maintainingquality and standards in Profession-Specific teamcomprises Health and Social Care staff, registered and non-registeredprofessionals, working towards integrating their approach in early supportivedischarge situations and complex hospital post holder works as assessor, responder and reviewer and providescomprehensive assessment and intervention planning, aiming to enable andempower people to remain in their own homes, or be cared for as close to home aspossible, while continuing their an experienced Nurse you will use your broad rangingskills and capabilities, undertaking an active role in assessing, planning,coordinating, delivering, developingand evaluating care to patients referred to will supervise and manage performance of any staffline managed by this post and provide leadership for support staff through supervision and appraisal.

Job responsibilities

Thepost holder will be clinically responsible for the provision ofquality nursing for the Discharge toAssess (DTA) Team who are referred from the acute general hospital setting. Those referred may experience a widevariety of health conditions and social care situations.

To work as a member of a Multi-professional Team, deliveringassessments and intermediate care ensuring high standards of clinical practiceare set, monitored and reviewed.

As a worker in DTA you will use broad ranging genericskills and capabilities while maintaining quality and standards inProfession-Specific skills.

The teamcomprises Health and Social Care staff, registered and non-registeredprofessionals, working towards integrating their approach in early supportivedischarge situations and complex hospital discharges.

Thepost holder works as assessor, responder and reviewer and providescomprehensive assessment and intervention planning, aiming to enable andempower people to remain in their own homes, or be cared for as close to home aspossible, while continuing their recovery.

You will liaise with planned resourcesbothinternal and external to the organisation and you will work inpartnership to offer optimum opportunity for people to meet their desiredoutcomes.

As an experienced Nurse you will use your broad rangingskills and capabilities, undertaking an active role in assessing, planning,coordinating, delivering, developingand evaluating care to patients referred to DTA.

You will supervise and manage performance of any staffline managed by this post and provide leadership for support staff through supervision and appraisal.

Youwill maintain electronicrecord keeping and you will haveopportunities for mobile working.

You will be able to demonstrate a high level of communication skills in terms of use of verbal, electronic and written formats.

You will be required to engage with patients who can bechallenging and /or acutely distressed.

Post holders will be expected to participate in theshift patterns over a 7 day period including weekends and bank holiday.

Professional /Clinical Responsibilities

The Post-holder will:

Work autonomously and unsupervised in the community setting, recognisingthe risks associated with loneworking, aggressive behaviour, and workproactively to reduce those risks.

To provide holistic evidence based/ bestpractice care to patients in accordance withNational and organisationapproved policies/procedures and individual care plans, examplesinclude hospital passport, Continuing Health Care decisionsupport tools and fast tracks.

To be responsible for the assessing,planning, implementation and evaluation of patient care on a day-to-day basis,as part of a multi professional team. The work may be unpredictableand complex and often of a sensitive nature.

To problemsolve and liaise as soon as possibleto the relevant clinician problems that may arise.

To be involvedin discussions around referralsand discharge planning.

To undertakecomprehensive risk assessments of all situationsassociated with the care of Patient's in order to ensurethe safety of patients' carers and team members. Thismight include the assessment and provision of equipment ensuring it is usedsafely.

Assessand prescribe communityequipment within clinicalcompetency, with awareness of budget requirements.

The post holderis required to complywith all infection control policiesand guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanationreceived, then it is theindividual's responsibility to escalate to Director of Operations/ Chiefexecutive office)

To monitor and maintainthe health, safetyand security of self and others.

To be conversant with the vulnerable adult and child protection procedures and policies of theorganisation.

To be responsible for organising own time management on a daily basisin line with caseload demandsbalancing patient needs with the administrative aspect of the role. Andescalating concerns appropriately.

The post holder will be required to use a computer, beingresponsible for timely recording patient activities for IT data collection,dealing with e-mail queries, stock and equipment ordering.

To preserve confidentiality and be aware of the Data Protection Act, Access to HealthRecords and Consent for Treatment.

To have a flexibleapproach to the working day in order to meet the needs of the patient/service.

Toreport to the line manager any untoward incidents complaints, accidents,defective equipment and take appropriate action to ensure safety of patients, relatives, self and colleagues.

Education& Training:

To maintain own professionalexpertise by attending meetings, study days and in-service training and learnnew specialist skills required for service development.

Experience

Essential

2 years + recent experience as a Band 5 Nurse either working in acute or community setting Ability to demonstrate transferable skills. Experience or understanding of community working. Relevant clinical experience and competence including: medicines management. Complex care co-ordination .

Desirable

Experience of inter-agency and partnership working. Supporting self-care, self- management and enabling independence. Managing care at end of life.

Knowledge and Skills

Essential

Evidence of post registration development. General understanding of Continuing Healthcare and the assessment process for this. Time management. Ability to manage environmental stress. Background knowledge of Government policies relating to intermediate care and home first. Understanding of Clinical Governance Current and up to date knowledge of The Code (NMC) Excellent interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers and the public. Excellent written and verbal communication skills. Ability to work autonomously without supervision Ability to adapt to the demands of a constantly changing environment.
