About the role

LIFELONG LINKS YOUTH SUPPORT WORKER APPRENTICE


The role is to support children and young people’s engagement during Lifelong Links work as well as promoting the Lifelong Links Project. 


The Lifelong Links Project is a new project recently launched at the LB of Hammersmith and Fulham. The project is offered to young people in care and care leavers.


Lifelong Links aims to find family members and other people who care about the young person such as former foster carers, teachers or family friends, who have been lost through their journey through care.

These people are brought together to reunite with the young person in a meeting called a Family Group Conference. At this meeting they can talk with the young person about what support they are able to offer, both now and in the future. This is written into a support plan for the young person, so that the young person has people in their lives, who love them, with the hope to improve their long term outcomes in life.


To support to raise the profile of the Lifelong Links process both internally and with our partner organizations. 


You will also complete an 18 month Level 3 Youth Support Worker qualification which will enhance your knowledge, skills and abilities and help you progress in your future career.
