Job summary

We are looking foran experienced, enthusiastic, friendly and professional Practice Nurse to join our team atPontesbury & Worthen Medical Practice on a permanent contract basis. Applicants must have experience of working in a GP Practice, and must have completed their fundamentals training.

The successfulapplicant would be working alongside our current nursing team, and be responsible for the deliveryof practice nursing services to our patient population. The role will includemanaging areas such as health promotion, chronic disease management, healthprevention, as well as supporting the nursing, clinical, and management teamsin achieving QOF targets.

Main duties of the job

We are looking forsomeone with good primary care experience, and an enthusiastic teamplayer who will, as well as being patient focused, will work well with thewhole practice team and maintain a friendly but professional conduct at alltimes. We are a hard working and friendly team who pride ourselves ondelivering excellent patient care.

Pontesbury & Worthen Medical Practice is a developing and growing practice with two sites, situated in the rural villages of Pontesbury and Worthen, just south of Shrewsbury providing medical care for approximately 10500 patients. We are a forward thinking team, keen to adopt and progress innovative ideas and solutions whilst providing excellent clinical care. We are a Dispensing practice. We are a training practice.

This list is not exhaustive and will flex and change to meet the demands of the practice, and with the successful candidate depending on what skills they currently hold:

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health Implement and evaluate individual treatment plans, manage chronic disease registers and run clinics for chronic disease patients Provide daily routine nursing care to patients as required in accordance with clinical-based evidence Provide wound care and dressings for patients Maintain accurate clinical records Ensure records and consultations are coded and recorded effectively and accurately Chaperone patients where necessary Support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs Implement and be active in running vaccination programmes for adults and children Support the clinical team with all safeguarding matters, in accordance with local and national policies Deliver opportunistic health promotion where appropriate Practice nursing duties including venepuncture, BP, ECG, injections and health promotion clinics Administrative duties including ordering, stock control and keeping rooms stocked with equipment. Undertake visits to our housebound patients or those in Learning Disability nursing or residential care, to administer Flu or COVID vaccines. Person Specification

Essential

Registered Nurse At least 2 years experience of working in a GP Practice, and must have completed the Fundamentals training

Postgraduate diploma or degree (Primary Care)

Experience of working in a GP Practice Experience of EMIS or other practice clinical system Excellent communication skills (written and oral) and interpersonal skills Strong IT skills and competent in the use of Office and Outlook Effective time management (planning and organising) Ability to work as a team member and autonomously Problem-solving and analytical skills Ability to follow clinical policy and procedure
