Job summary

Thepost holder is an experienced nurse, who has either graduated from a Centre forAdvancing Practice accredited MSc advanced practice programme or completed theCentres ePortfolio (supported). They will act within their professionalboundaries, will provide care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care. Theywill demonstrate safe, clinical decision-making and expert care, including assessmentand diagnostic skills, for patients within general practice. The post holderwill demonstrate critical thinking in the clinical decision-making process.They will work collaboratively with the general practice teams to meet theneeds of patients, supporting the delivery of policy and procedures, andproviding nurse leadership as required. In order to work at this level NMCrequirements for advanced practice must be met. They must also be anindependent prescriber.

About Healthy Crawley PCN

Healthy Crawley PCN is a NHS Collaboration between 4 GP Practices Furnace Green Surgery,Ifield Medical Practice, Poundhill Surgery and The Woodlands/ClerklandsPartnership - working together to provide enhanced access services.

Our PCN builds on the existing primary careservices and enables a greater provision of proactive, personalised and moreintegrated health and social care. We are supported by practitioners inadditional roles who allow us to create bespoke multi-disciplinary teams basedon the needs of our local population. ClinicalPractice

Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis. Clinically examine andassess patient needs from a physiological and psychological perspective.

Assess,diagnose, plan, implement and evaluate interventions/treatments for patientswith complex needs. Proactively identify, diagnose and manage treatment plansfor patients at risk of developing a long-term condition.
