Job summary

We looking for qualified or trainee Advanced Clinical Practitioners with non-medical prescribing and advanced adult health assessment qualifications to join our GEMS (Geriatric Emergency Medicine Service). GEMS has an enthusiastic and motivated multi-disciplinary team of Advanced Clinical Practitioners, Pharmacists, Nurses, Physiotherapists, Occupational Therapists, and Social Workers who work alongside Consultant Geriatricians to deliver Comprehensive Geriatric Assessment at the busy front door of the acute hospital and in-reaching to our inpatient areas to embed a frailty sensitive ethos.

Currently/soon to be engaged with e-portfolio supported route for Advanced Practice or in 2nd year of ACP we would love to hear from you.

Main duties of the job

Job description

Job responsibilities

Provide care for older patients who have acute physical illness combined with other problems including co-morbidities and needs, functional abilities and rehabilitation (Geriatric Giants/Frailty Syndromes).

Work towards health promotion and prevention and comprehensively assess patients for risk factors, early signs of illness or deterioration in condition.

Draw on an advanced clinical knowledge and skill set to diagnose and plan treatment and care, which will include ordering and interpreting diagnostic tests, advanced health assessment and cognitive assessment.

Prescribe or deprescribe medication where legally allowed and actively monitor the effectiveness of therapeutic interventions.

Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.

Work in partnership with patients, families and carers to support them to manage and live their lives.

In addition they will help to develop a programme of practice development for staff in the Frailty Pathway who will also be caring for these patients. Their practice should be developed to an advanced level whereby the Advanced Clinical Practitioner can directly or indirectly influence all aspects of care and management of patients within their speciality, and across a population.

Person Specification

Qualifications

Essential

Experience working in specialist clinical services to Acute Medicine/ Frailty/Geriatrics Speciality. Experience in clerking and undertaking independent examination of patients; diagnosing and treatment planning. Experience of successfully supervising and contributing to the training of own professional team and proven experience of training nurses, doctors and other healthcare professionals. Experience of autonomous practice. Experience in providing an on-call services.

Desirable

Published in professional journal or presented at conference/equivalent. Experience/Involvement in quality improvement initiative.

Essential

BSc (Hons) Degree in Nursing/Pharmacy/Physiotherapy/Parame dic Science MSc Advanced Clinical Practice. Registration with the relevant professional body; Nursing & Midwifery Council, General Pharmaceutical Council; Health & Care Professions Council. Postgraduate clinical specialist knowledge acquired through diploma level training Diploma in Clinical Pharmacy or equivalent. V300 independent prescribing.

Credentialed by the RCN. Member of the RPS Faculty. Member of the UKCPA. Member of Chartered Society of Physiotherapy. Member of the College of Paramedics. Leadership & Management Qualification. Quality Improvement qualification. Teaching Qualification.
