Job summary

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England. We have a fantastic opportunity for an Advanced Clinical Practitioner to join our team in the Out of Hours clinical hub in Denton, Manchester.

Our clinical hub is open 24 hours a day, 7 days a week and we have a variety of shift opportunities available for both full time and part time working, predominantly working evenings and weekends.

Whilst working within our clinical hub, you will work alongside a multi skilled team that includes Advanced Clinical Practitioners, Senior Clinical Practitioner, Registered Nurses, Dental Nurses, Prescribing Pharmacists and GPs.

As an ACP you will work within your professional boundaries and assess, diagnose and prescribe where appropriate. You will provide advice, care, and management for urgent and / or primary care patients predominantly via telephone and video or face to face in treatment centers or patients own homes.

The successful candidate will support where necessary, Clinical Practitioners and Senior Clinical Practitioners in their clinical decision making and management of patients who present with undiagnosed and undifferentiated illness and injuries.

Main duties of the job

The post holder will hold an MSc in Advanced Clinical Practice or Credentialing qualification (or equivalent) and a Non-Medical Prescribing qualification. You will be able to work within the four pillars of practice.

Within your professional boundaries you will assess, diagnose and prescribe where appropriate. You will provide advice, care and management patients presenting with undiagnosed or undiagnosed undifferentiated illness. Your role will include supporting the delivery of policy, procedures and leadership as required.

The clinician will act as a positive role model supporting junior staff. The successful candidate will demonstrate clinical reasoning, critical thinking and will be key to supporting clinical work across the healthcare team to meet the needs of patients. You will participate in and lead change for the benefit of patients and/or the organisation and be a key member of the senior clinical team.

Job responsibilities

ClinicalPractice

Assessand holistically clinically examine patients with both minor and complex needsthrough history taking, physical examination and appropriate use ofinvestigations to confirm diagnosis. Plan,implement and evaluate an effective management plan for the patient, ensuringsafe discharge/referral and/or transfer to an appropriate health care service. Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions. Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare. Referand discharge safely ensuring that safety-netting advice is offered. Triagetelephone referrals, calls from external clinicians, patients and others into arange of services appropriately as required.

Communication

Utiliseand demonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment. Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating. Anticipatebarriers to communication and take action to improve communication. Maintaineffective communication within the organisation and with external stakeholders. Ensureawareness of sources of support and guidance and provide information in anacceptable format to all patients, recognising any difficulties and referringwhere appropriate

Non-Medical Prescribing

If you are employed in a prescribingcapacity, you must only prescribe within your scope of practice and comply withthe gtd healthcare NMP governanceregistration processes. You must maintain your knowledge and skillsand prescribe within your scope of clinical competencies. The non-medicalprescriber is required to comply with organisational NMP governance processesand adhere to local and national prescribing guidance.

Delivering a Quality Service

Recogniseand work within own competence and professional code of conduct as regulated bythe NMC/HPC etc. Understand own role and scope and identify how this maydevelop over time. Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures. Prioritise,organise and manage own workload in a manner that maintains and promotesquality. Ensureprofessional revalidation requirements are met. Delivercare according to NSF, NICEguidelines and evidence-based care. Assesseffectiveness of care delivery through self and peer review, benchmarking andformal evaluation. Initiateand participate in the maintenance of quality governance systems and processesacross the organisation and its activities. Inpartnership with other clinical teams, collaborate on improving the quality ofhealth care responding to local and national policies and initiatives asappropriate. Understandand apply guidance that supports the identification of vulnerable and abusedchildren and adults, and be aware of statutory child/vulnerable patient healthprocedures and local guidance.

Qualifications

Essential

Registered health care professional, including first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent. V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.

Desirable

Mentor/Teaching qualification. Clinical supervision training and experience.

Experience

Essential

Experience of working as senior clinician. Experience of autonomous practice. Experience of leading/managing quality improvement.

Desirable

Experience in leading/managing a team. Experience of working in an unscheduled care service/primary care. Experience of having worked in Primary Care.
