Job summary

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England. We have a fantastic opportunity for a Clinical Practitioner to undertake telephone triage based within our Out of Hours clinical hub in Denton, Manchester.

Telephone Triage Clinicians provide a comprehensive triage service, for patients requiring out of hours services, providing appropriate information, advice or referrals, as per the agreed protocols and pathways.

Whilst working within our clinical hub, you will be part of a multi skilled team that includes Advanced nurse practitioners, Senior Clinical Practitioner, Registered Nurses, Dental Nurses, Prescribing Pharmacists and GPs.

Our clinical hub is open 24 hours a day, 7 days a week and we have a variety of shift opportunities available for full time and part time working including days, evenings and weekend work.

We would welcome applications from capable, pro-active, and experienced Nurses, Paramedics and Pharmacists. No experience of telephone triage is required as full training will be given.

Main duties of the job

The clinical practitioner will function as part of a health care team providing telephone triage and a comprehensive clinical assessment service to patients and/or their carers/relatives resulting in a safe and appropriate clinical decision.

You will facilitate the provision of accurate healthcare information, health improvement advice or, where applicable, referral to the appropriate level and location of care, in order to meet the holistic and therapeutic requirements of the patient/carer.

The successful candidate will work within codes of practice and professional guidelines.

Job responsibilities

Service Deliveryand Performance

Act asa professional role model at all times. Developand maintain expertise, practice and competence in the role of ClinicalAssessor Provideskilled and effective Face to Face / telephone / Video assessment for patientspassed from a range of services such as 111 / work aspart of multidisciplinary team within the urgent care / primary care / OOHs setting The ClinicalPractitioner will undertake a telephone / video consultation using the computersoftware system and will always use professional judgement. Toapply critical analysis along with a high degree of clinical curiosity Empowerpatients to enable them to act upon advice given and embrace self-management. Accuratelydocument consultation notes in a timely fashion to allow for suitable emergencyreferral and intervention. Toshare and utilise areas of specialist practice with peers and colleagues toinform and enhance patient care. Towork with the management / operational team to contribute to the developmentand delivery of service. Identifyand use information sources such as the Directory of Services (DOS) to supportand underpin clinical decision-making. Providehealth education and self-management advice during the consultation process. Referto the ACP Lead / Lead SCP / medical Lead as necessary for advice. Facilitateand actively participate in the continuous quality improvement audit process. Reflectas necessary on this process. Toattend and contribute to organised meetings. Tofollow gtd healthcares policies and procedures, , the failedencounter procedure and the home visiting policy. Understandand embrace local pathways including use of ED for urgent and immediate needs.

Effective Communication

Activelycommunicate with all staff. Activelycommunicate with other service providers about patient care , childprotection issues. Workwithin the governance agenda contributing to service improvement. Reflectthe diversity of needs in the local healthcare community by utilising all availableresources to facilitate access to the service where there are barriers tocommunication and understanding. Utilise advancedlistening skills, probing and facilitative skills across a diverse range ofcalls Face to Face assessments, some of which are highly challenging due toemotive circumstances or patient aggression. The post holder shouldrecognise the importance of effective communication within the team and willstrive to communicate effectively with team members, patients, and carers Utilisesenhanced communication skills to negotiate and provide support to service userswho may be non-compliant with recommended outcomes whilst adhering to the relevantgtd healthcare policies and procedures.

Management of Risk

Maintaina healthy and safe work environment for self and colleagues. Haveresponsibility and accountability for the escalation of any adverse incidentsor events to the appropriate persons both internally and externally, whilstensuring that the appropriate documentation is recorded, and that the ACP lead/ operational lead is actively involved and communicated with.

Clinical Duties

Assess patient both adults and children need from a physiological and psychological perspective and plan care accordingly in a timely manner. To work with members of the MDT and independently to assess, plan, implement and evaluate interventions/treatment for patients with complex needs and care for patients. To work with members of the MDT. Provide health education and promote healthy lifestyle for patients and carers. Provide clinical and professional advice. Promote safe systems of work and identify areas where changes in Urgent Care / primary care would achieve a more effective outcome. Ensure resources are used efficiently and effectively and all equipment used in the care of patients is appropriately maintained and used only by those individuals with the appropriate training.

Qualifications

RGN or equivalent professional qualification. Current NMC, HCPC, GPhC Registration or equivalent.
