Job summary

A vacancy has arisen for a Nurse Associate to join our Community Nursing Team at The warrens Community Nursing Team, based in Warrens Medical Centre to work within the Wirral Community Health and Care NHS Foundation Trust.

hours per week , Working shifts and over 7 days .

It is a permanent post.

Successful candidate must be flexible in there approach to working shifts and weekends .

Main duties of the job

You must have the Nursing Associate Foundation degree and be on the NMC Register .

We are looking for someone who is skilled in assessing and planning nursing care.

The post holder acts professionally at all times and uses their knowledge and experience to make evidence based decisions and solve problems. They recognise and work within the limits of their competence and are responsible for their actions. They contribute to the provision of care for people, including those with complex needs. They understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family, and their responsibilities in relation to communication and collaboration.

Our Community Nursing Team supports the needs of housebound patients living on the Wirral. We provide valuable care, supporting people to manage their long term conditions, and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they admitted.

The post holder will improve the quality of care by contributing to the continuous monitoring of people's experience of care. Identify risks to safety or experience and take appropriate action, putting the best interests, needs, and preferences of people first. They will contribute to on-going assessment and can recognise when it is necessary to refer to others for reassessment. The role requires the ability to be flexible and adaptable whilst working in people's homes and other community settings. The post holder will have some responsibility for supervising less experienced or qualified staff and students in community placements. They will be expected to actively contribute to quality assurance processes and service development.

Qualifications and Profession

Essential

Registered Nursing Associate on NMC register Nursing Associate Foundation Degree GCSE Grade A-C Maths and English or skills level 2 Maths and English or equivalent

Experience and knowledge

Essential

Understanding of the scope of the role of the NursingAssociate in context of the nursing and interdisciplinary teamand the organisation, and how the role may contribute to service development Understands and acts in line with NMC professional standardsfor practice contained within Standards of Proficiency for Nursing Associates Understand requirements for NMC professional Revalidation Knowledge of a broad range of conditions experienced by patients in the community and general practice settings. This will include Long term Conditions and common mental illnessess,frailty and palliative and end of life care. An understanding of the presentations of multiple pathology,depression and anxiety states and fraility, predominantly in older people Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice . Understand the importance of following procedures and treatment plans Experience of working in teams under appropriate supervision as part of a multi-disciplinary team Insight into how to evaluate own strengths and development needs , seeking advice where appropriate Experience of providing and receiving complex, sensitive information
