This is an exciting opportunity to be part of the joint venture between Wirral Community Health and Care NHS Foundation Trust and Wirral University Teaching Hospital where you be supporting people to remain in their own homes, under the guidance of registered nurses, physician associates and medics.

Care is provided to patients within their own homes to prevent unnecessary hospital admissions.

**To join our multidisciplinary team you will**:

- Have an NVQ Level 3 in Health and Social Care, or demonstrable equivalent experience
- have a caring attitude
- have a strong desire to help people
- have good communication skills

The post involves a 7 day working rota.

About the Community Integrated Response Team (CIRT)

CIRT is the team name for several services including Urgent Community Response, Home First and Frailty Virtual Wards, which includes; Nurses Physiotherapists, Occupational Therapists, Social Workers, Physician Associates and Medics. The team have a strong focus on admission prevention and early supported discharged from ED and wards. One of the fundamental principles of the Healthcare Assistant Role is to support our patients with their to remain safe at home.

To provide and assist with the care of people in their homes, working with individuals and their families to ensure a person-centred approach including the participation in clinical care procedures e.g recording observations and basic wound care.

To support admission prevention by responding to incidents of carer breakdown, provide rapid support for individuals to prevent admission to hospital and liaise with nursing colleagues on support plans.
