Job responsibilities

1. Job Purpose:

Contribute to the provision of a clinical perfusion service for the Trust, incorporating a primary responsibility to the Hammersmith Hospital anda peripatetic clinical service to the Trust in general. This will includecardiopulmonary bypass, cell salvage and intra-aortic balloon pump systems.

Highlevel of competence in performing a broad range of routine adult clinicalperfusion techniques and procedures. Contributeto the maintenance of standards and quality in perfusion services. Participate in 24/7 on-call perfusion servicewithin the Trust.

4.General responsibilities

ClinicalPractice

To comply with the Code and Standards of Practice asdefined by the Society of Clinical Perfusionists of Great Britain and Ireland. To take, withappropriate delegation, responsibility for the delivery of a highly specialisedperfusion service across the Trust including cardiopulmonary bypass, intra-aorticballoon pump and cell salvage procedures, with an adaptable approach to ensurea flexible and effective service is maintained. To demonstratespecialist skills, knowledge and expertise in both Adult clinical practice.Applying these skills to the supervision and training of others throughout thedepartment and the multi-disciplinary team. Draw on knowledge ofa range of theories to inform practice. Assist in developing strategies for themanagement and delivery of care for the client group. Setting up, primingand running bypass equipment for Adult cardiac surgery procedures, according tothe perfusion procedures and protocols and to play a full role in the routinecardiac surgical programme. Ensure qualitycontrol, calibration and maintenance of expensive and complex medicalequipment, ensuring accurate and safe use during clinical procedures. Assess the patientfor relevant medical history, pathology and diagnosis and make appropriateclinical decisions concerning the conduct for a range of Cardiac proceduresincluding Adult and complex congenital. To administer drugs,according to local policy and as necessitated by the clinical situation priming solutions, anticoagulants, inhalational anaesthetic agents, potassium,and cardioplegic solution Participate in the on-call rota to provide 24/7perfusion cover within the Trust including trauma at St Marys Hospital in order to meet demand. Be fully conversantwith specialist techniques and perfusion procedures as required by individualpatients such as, cerebral perfusion, vacuum assisted venous drainage, IABP andblood conservation techniques. Ensure that theperfusion department is able to assist in the insertion and management ofintra-aortic balloon pumps as required by individual patients throughout theTrust. Encourage, implementand take a full role in departmental and individual projects, with a commitmentto research based clinical practice. Contribute toevidence-based clinical practice to maximise clinical effectiveness. Liaise with othermembers of the Perfusion Team/Service Manager and medical staff agreeingworkload for the unit in order to meet contractual requirements advising ofproblems in advance. Initiate andcontribute to the development of and implementation of protocols and proceduresas relating to perfusionist duties, developing and reviewing documentation,according to the policies and practice committees. Participate in ensuring quality control,calibration and maintenance is in place for the relevant specialist perfusion/bloodmanagement equipment and intra-aortic balloon pumps. Maintain accurate perfusion records duringclinical activity. During the operative procedure exercise extremevigilance of the patients haemodynamic status with accurate and appropriateinterpretation of the wide range of data outputs. On assessment of thesethroughout the procedure, respond to changes and deliver concise accurateinformation to the surgical and anaesthetic team Keep up-to-date with developments withinperfusion, undertaking additional training as appropriate.

Qualityand Service Development

Be proactive in the updating and development ofdepartmental Standard Operating Procedures taking into account current evidenceand clinical governance to optimise clinical outcomes. Contribute to projects for perfusion-relatedservice development within the context of multidisciplinary team settings andservice delivery plans. Participate in ensuring adequate policies arein place to protect the patient from risk, for example, by helping prevent thespread of infection. Contribute toeffectivecommunication systems that are established throughout the Directorate andacross the Trust, ensuring that all communication is relevant and appropriate.

Policies/ Service Development / Implementation

Deliver, withappropriate delegation, perfusion team objectives and projects to ensure aseamless delivery of service across both clinical sites. To negotiate withothers around service issues and caseload management. To facilitateinvestigation into complaints, accidents / incidents made in respect of thePerfusion Department, feeding back to the Chief Perfusionist To facilitate themanagement of specific clinical issues and ensure appropriate communicationpathways are developed and evaluated across the service. To support staff toimplement team objectives and projects. To contribute to,develop and monitor that protocols are in place for each specified area of theclinical perfusion service. To advise perfusionmanager on issues of service delivery including shortfall, service pressuresand activity. To assume delegatedtasks as requested by line manager, including leading on policy developmentwithin area of clinical expertise. To support thedevelopment of specialised care protocols relating to a range of issues toimprove patient care blood conservation techniques employed within theTrust.

Financialand Equipment Resources

Participate with the procurement of perfusionrequirements ensuring adequate equipment is available when required andensuring that budgetary responsibilities of the service are managed to avoidadverse financial situations. To support a perfusion equipment profile, ensuringthat all equipment is in safe working order and regularly maintained withadequate records of work undertaken. Appropriate action should be instigatedwhere equipment is found to be faulty. Avoid waste, identify cost pressures andopportunities for saving to help ensure service provision is within theallocated budget.

Information Resources

Accurately collect clinical data usingappropriate software. Contribute to the maintenance, development andreview of appropriate activity records and information for audit, finance,equipment and performance management, professional registration andbenchmarking purposes. Support the Chief Perfusionist in monitoringappropriate data for service benefit, identifying trends and makingrecommendations. To maintain up to date and accurate patientcase notes in line with Society of Perfusionists of Great Britain and Irelandand professional standards and local Trust policies.

Audit andResearch

The post holder is expected to participate inthe Trusts clinical effectiveness activities, and to maintain and fosterimprovements in the quality and standards of clinical services. In conjunction with the Chief Perfusionist,assist the Directorate team to meet clinical governance targets and contributeto reporting processes. Contribute to the development and delivery ofresearch opportunities relating to the practice of perfusion support andcontribute to the Imperial College academic and service development programmes. To participate in research activities and helpdetermine suitable methodology for research or service development projects. Ensure that completed research is presented tothe wider perfusion community.
