Job summary

The core purpose of this role is to work across the older peoples' inpatient mental health service to embed the Occupational Therapy assessment and intervention pathway across the service within the framework of MOHO, in collaboration with the wider MDT staff.

The post holder will function as part of the MDT to deliver the Interventions required to support individuals through their recovery pathway and equip them with the skills and confidence to enable a safe discharge from the service and to support assessments and decision making within the MDT.

The post holder will be highly skilled and competent in conducting OT assessments developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures.

The post holder will provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members.

The OT will be responsible for developing treatment plans and intervention to be delivered by the team's Technical Instructor and Activity Co-ordinator.

Main duties of the job

o To be responsible for the assessment treatment and evaluation of patients referred specifically for OT intervention from the inpatient To be able to work across all functions within the inpatient service and support patients to achieve their individually agreed To work as part of a multi-disciplinary team to, assess, plan and implement individualised programmes of care/interventions in accordance with CPA (where appropriate).

o To assess OT need and plan therapeutic OT interventions using the Model of Human To lead on the development of group based interventions to aid recovery and promote social To facilitate the development of peer support networks as part of the discharge pathway from the service.

Job description

Job responsibilities

To deliver a range of therapeutic interventions in line with professionalrequirements and in accordance with the job specification. To write reports using MOHO outcome measures and to ensure up-to-date and accurate records are maintained regarding all patient contact. To use a comprehensive range of verbal and non-verbal communication tools to communicate effectively with patients, their carers and families and may include people who may have difficulties with understanding orcommunication. To be responsible for managing a caseload of clients as required. To manage patient discharge in a safe and timely manner, ensuring liaison with relevant professionals, to ensure that family and carers are, where appropriate, advised of the patients discharge arrangements, with timely completion of documentation. To develop partnership working with all relevant agencies/services/individuals to support optimal care for patients. To work in partnership, direct, support and supervise the teams TechnicalInstructor and therapy support workers.

Person Specification

Education / Qualifications

Essential

Must hold a diploma or degree in Occupational Therapy and be registered with HCPC. Evidence of post qualification experience and continuing professional development to post graduate diploma level. Willingness to undertake further training relevant to the role resulting from legislative or service changes, as appropriate.

Experience

Essential

Substantial post registration experience working in a mental health service. Experience in working with older people with severe and complex mental health needs. Experience of MDT working.

Desirable

Experience of managing or participating in change. Experience of leading a team. Experience of supervision. Experience of managing a caseload. Experience of working with older people.

Knowledge

Essential

Extensive knowledge / experience in social inclusion and recovery model of care. Sound understanding of current local and national policies in relation to Mental Health including the Mental Health Act, Mental Capacity Act, Risk Assessment and Management processes and CPA processes. Good working knowledge of assessment, care planning, implementation and evaluation processes. Comprehensive knowledge of mental health conditions and how they impact on an individual's wellbeing and occupational functioning. Comprehensive knowledge of broad range of interventions required to support older people with severe and complex mental health needs.

Desirable

Knowledge of MOHO and its' assessment tools. Ability to carry our cognitive assessments. Knowledge of the occupational context of inpatient mental health services and it's application to assessment and treatment.

Skills and Abilities

Ability to be responsible for a caseload. Ability to work effectively as part of a MDT. Ability to manage and prioritise own workload. Ability to demonstrate specialist expertise in assessment using MOHO. Ability to deliver a range of evidence based interventions on both an individual and group basis. Ability to supervise junior staff/students. Effective time management. Excellent written and verbal communication skills. Recording relevant documentation to a professional standard. Ability to communicate with and develop supportive relationships with patients, carers and other professionals Ability to value diversity and respond constructively to discriminatory behaviour. Ability to provide professional expertise in the care of mental health service users Ability to provide appropriate education and training to peers and assistant as appropriate. Computer literacy. Flexible and adaptable.

Desirable

Interest in engaging in OT service development with the recognised Trust work streams. Ability to audit outcomes of assessments and OT service provided. Interest in developing the OT role in older peoples' services.

Work Related Circumstances

Essential

Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs.
