Job summary

Weare looking for a Practice Nurse who has a special interest in Diabetes to joinour friendly and proactive dispensing practice for 10 hours a week.

Main duties of the job

The successful candidate will deliver safe and effective nursingcare to our patients and working in accordance with the NMC. The post holderwill provide a holistic approach to patient centred care and with a particularinterest and / or experience in managing patients with Diabetes.

You will be responsible for reviewing their treatment,blood test results and providing education and health management to optimisetheir health and wellbeing.

About us

We have an excellent, dedicated and loyal team working from purpose-built premises with a list size of approximately 6000 patients. We use TPP SystmOne. Working with 5 GPs, 3 PN's and 3 HCA's.

JOB DESCRIPTION

JOB TITLE: DIABETIC NURSE

REPORTS TO: THE PARTNERS/LEAD NURSE (Clinically)

PRACTICE MANAGER (Administratively)

HOURS: 10 hours per week

Job Summary:

The post-holder will work with reference to Practice agreed protocols and guidelines to provide diabetic nursing support to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will maintain the required level of training and competence and will work within patient group directions where these are available.

The post holder will liaise where necessary with the Department of Diabetes at Poole General Hospital OR Dorset County Hospital.

Main Duties:

Management of Patients with Diabetes Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals Therapeutic Monitoring: Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach Recognising abnormalities Identifying the impact of treatment and implementing or recommending changes as appropriate. Attending patients at home if necessary.

Health & Safety:

The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health & Safety Policy, to include:

Using personal security systems within the workplace according to Practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Reporting potential risks identified.
