Job Title: Diabetes Specialist Nurse

Job Summary: As a Diabetes Specialist Nurse at Purbeck PCN, you will provide education and support to healthcare professionals, offer telephone advice, and see patients in clinics or through home visits. You will collaborate with Dr. Masding Consultant Diabetologist to ensure patient education and treatment changes.

Main Duties:

  • Work as a Specialist Diabetes Nurse covering all 6 GP practices in Purbeck.
  • Provide telephone advice to healthcare professionals and accept diabetic patient referrals.
  • Enhance education for clinicians and contribute to achieving PCN objectives for diabetes care.
  • Report to the PCN GP Diabetes Lead & PCN Clinical Director.
  • Receive support for role development and skill enhancement.

About Us: The Purbeck Community Diabetes Service has seen significant improvements in diabetes care over the past 18 months. Join us to further enhance diabetes care for Purbeck residents and support Diabetes Practice Nurses in managing complex patients.

Job Description:

Job Responsibilities: Join the Purbeck PCN to deliver specialist diabetes care, offer advice to healthcare professionals, and accept patient referrals to enhance diabetes care.

Travel between GP practices in Purbeck, with Wareham Surgery as the base. Occasional home visits may be required.

Main Duties:

  • Provide high-quality diabetes care from diagnosis to long-term management autonomously.
  • Promote self-care management for patients' health improvement.
  • Stay updated with current guidelines and professional development.
  • Ensure QOF compliance and achieve targets.
  • Collect data to show patient improvement and monitor relevant data.
  • Deliver best practices for clinical care, education, and research.
  • Act as a specialist resource for diabetes-related advice.
  • Collaborate with other healthcare services for patient access and care.
  • Identify high-risk patients for intensified treatment.
  • Enhance services for housebound and care home patients.
  • Liaise with secondary care diabetes services and create treatment plans.

Person Specification:

Experience

  • Essential: 2 years post-registration experience in managing patients with diabetes.
  • Desirable: Previous experience in Primary Care, clinical audit, and training environments.

Qualifications:

  • Essential: NMC registered adult nurse, Diploma in diabetes-related field, UK driving license.
  • Desirable: Non-Medical Independent Prescriber, Advanced Clinical Practice MSc.
