This is a fixed term contract for 12 months from the recruitment date. 

There are currently 2 X Target 1000 Care Coordinator vacancies and the work will be divided across Newham, Hackney, Tower Hamlets, City of London, Waltham Forest, Redbridge, Havering and Barking and Dagenham.

North East London Integrated Care System has secured additional funding through the Rough Sleeping Initiative to “get people off the street more quickly, secure move on accommodation, help individuals to sustain their tenancies and provide bespoke support for those furthest away from leaving rough sleeping for good.” 

As part of this funding scheme, North East London (NEL) has secured funding to support a pilot project to provide personal health budgets for homeless people that fall within the T1000 cohort. This role is key to delivering this pilot and providing co-ordination across NEL.

This is an exciting opportunity to make a real impact on the most vulnerable rough sleepers across the North East London Boroughs. You would work closely with the Rough sleeping leads across the North East London to identify the most complex rough sleepers and devise a plan to get them of the street for good. With the allocation of personal budgets, you will be responsible for ensuring that a person centred plan is in place to support these complex individuals into a clear pathway off of the streets and help them maintain their accommodation..

You will work in collaboration with the Outreach Teams in each of the Boroughs you are responsible for as well as the Voluntary community and faith groups, Homelessness Prevention and Advice Service, various commissioned providers who deliver accommodation, mental health support, drug and alcohol support etc, other internal and external stakeholders to help the most identified complex rough sleepers on the streets across the NEL Boroughs.

You will be responsible for the health care budget assigned to each of the rough sleepers within this pilot. To co-ordinate activity in support of the project’s objectives, you will be provide project updates through the appropriate governance routes at a local, NEL and pan-London level including monitoring progress, reporting on PHB spend and capturing outcomes. 

You will be required to provide guidance and support to the Outreach Workers and convene borough Multi-Disciplinary meetings, to advocate for identified participants at high risk panels, report serious safeguarding cases as appropriate. 

You will work with other team members including the Homeless Prevention and Advice Service to ensure rough sleepers qualifying for assistance under the Homelessness Reduction Act 2017 are offered relief duty where appropriate.

You may be required to be carrying out outreach and offer support to rough sleepers by arranging appropriate support and accommodation to help prevent homelessness reoccurring. Your positive attitude towards your work and others will help to contribute to a great working environment. You will have a calm and personable manner and be able to handle potentially difficult situations. 

You will exercise due consideration to confidential information relating to the Council’s business by ensuring you do not act in a way which may be prejudicial to the Council’s interests. This applies to the general business of the Council and information gathered regarding individuals.

You will have excellent IT skills in a range of systems, a flexible approach and the ability to meet tight deadlines. You will need to demonstrate a professional approach and a strong commitment to customer services.

You will contribute towards the implementation of a working culture that is result-orientated, customer focused and an ethos of 'right first time' service delivery. Undertake all duties with minimum supervision and be responsible for continuous personal development by requesting and attending training where necessary.
