An excitingopportunity has arisen for a flexible Junior Nurse Practitioner to join ourRecovery At Home Team in South Tyneside, based in Clarendon in Hebburn.
The Teamconsist of an extensive MDT which includes nurse practitioners, nurses,occupational therapists, physiotherapists, health care assistants, therapyassistant/exercise practitioners and administrative staff. The Team also worksclosely alongside partner agencies including those from the local and voluntarysector.
This role wouldinvolve assessing people in their own homes who are either acutely unwell orhave recently been discharged from hospital and require enhanced interventionand short term support for their physical and functional health needs to remainat home.
The Service isfast-paced and you would be expected to see a wide range of patients whopresent with a variety of conditions.
Main duties of the job
The Service iscommitted to providing a sound environment for education and training tosupport you within this role.

NMC Adultregistered Nurse with significant post-registration experience. Degree or candemonstrate on a Degree pathway. AccreditedClinical Skills qualification In history taking and physical examination. Portfolio ofevidence of competence and continuous professional development.

Job responsibilities
PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE.
To provide care to people in their own homes to prevent admission/ readmission to hospital by providing rapid assessment of patients in the community over a 24 hour period. Undertake comprehensive holistic assessment of patients in both planned and unplanned situations, be able to prioritise clinical need, initiating investigations, determine a differential diagnosis and implement a plan of care reflecting evidence based guidance. To maintain agreed standards of care, integrity and dignity of all patients in an environment which is conducive to caring for those patients in their own homes. Undertake triage within the service determining clinical priority of calls, working with the coordinator ensure teams are dispatched to respond to the referral. Work collaboratively with multi professional teams to ensure practice is efficient, effective and evidence based. To develop his /her scope of professional practice in order to support the team to meet service requirements in accordance with professional guideline. To provide continuing responsibility in the absence of the Senior Nurse Practitioner and act in their absence. Person Specification
