South Tyneside 0-19 Team haveexciting opportunities for either a passionate & committed Health Visitor orStudent Health visitor who are due to qualify within the next3 months. Commencement in post at Band 6 will be subject to ability to provide evidenceof relevant qualifications. Successful applicants who are due to qualify willbe offered a conditional offer subject to providing evidence, this will be for3 months and will be paid at band 5 until qualification can be verified. We arelooking for candidates with excellent clinical & communication skills &those who are able to deliver safe & effective practice. Successfulcandidates will be responsible for the planning & delivery of the HealthyChild Programme to a diverse population ranging from universal in reach topersonalised in response. You will support the children & families of SouthTyneside have the best start in life. The post holder will also contribute tothe Public Health Agenda, meet Public Health targets & address local healthinequalities. The role requires an individual with a passion for working togetherwith multi-agency (statutory and voluntary) partners & providers to improvethe lives of children and families. Main duties of the job
To work unsupervised as a health visitor with caseload responsibility to influence the health and wellbeing of children and families to improve the life chances and give every child the Best Start in Life To be responsible for leading the universal national Healthy Child Programme through home visiting, clinics and multiagency settings Family Hubs To work with a wide range of partner agencies to support children and families in need, looked after children and children where there are child protection concerns To be the Lead Professional for assessment, early identification and care planning for vulnerable families, children with additional needs and the implementation of targeted specialist programmes of care Early Help Plans, postnatal depression and Solihull behaviour management Work in collaboration with local statutory, voluntary and mainstream services to ensure that children and families can access other health and support services To be responsible and accountable for the delivery of safe, effective and compassionate care
