Job summary

We are currently recruiting for a Community Nurse for people with Learning Disabilities and Forensic needs. The Post holder's key role is to work autonomously as part of the Adult Community Learning Disability Team Pathway alongside the Clinical Nurse Specialist Forensic Liaison Lead, Clinical Psychologist and Consultant Psychiatrist (sessional) to improve the quality of care received by people with a Learning Disability who have a forensic needs presentation (TCP/LD&A cohort) or are at risk of offending.

The post holder will ensure that care is delivered in the least restrictive environment with maximum integration into community.

The role will require the post holder to work in with people and their families to ensure person centred care and interventions and improved health outcomes. This will involve signposting (where appropriate) to preventative services and support, to advise and support in respect of reasonable adjustments where post holder will also contribute to the development of and the delivery of specialist training to staff groups.

Main duties of the job

Working as part of the Community Learning Disability Team to assess risk and provide advice, supervision, training and support regarding people with Learning Disabilities presenting with Forensic needs or at risk of offending. Avoidance of inappropriate hospital admissions and facilitation of timely discharge from hospitals.

The post-holder with support from the Clinical Nurse Specialist, Forensic Liaison Lead will work autonomously, and act as CPA co-ordinator for People with Learning Disabilities with forensic needs who are at highest risk of offending and/or admission to in-patient settings. They will provide therapeutic inputs, including collaborative safety planning where required. They will offer advice, support and clinical supervision to colleagues in MPFT and those working in other agencies ( police, probation) in relation to people with Learning Disabilities and forensic needs as appropriate. They will engage fully & autonomously in local processes to avoid inappropriate hospital admission for people with Learning Disabilities and forensic needs including contributing to the Dynamic Support Register (DSR) and CETR/CTR processes. The main focus of the role will be to ensure people with a Learning Disability are cared for in the least restrictive environment and that any risks in relation to forensic presentations are appropriately assessed and managed.

Job responsibilities

KEY RESPONSIBILITIES 1. Works autonomously across Staffordshire to ensure provision of an integrated healthcare service which includes highly specialist assessment to meet the forensics needs of people with Learning Disabilities.

2. To utilise specialist assessment skills to develop robust clinical formulations, care planning, therapeutic intervention/treatment and evaluation for people with Learning Disabilities and forensic needs who are part of the TCP/LD&A cohort.

3. To work autonomously with other agencies as appropriate to ensure multi-disciplinary assessment reports and recommendations are completed as required and that outcomes are routinely measured in line with agreed protocols.

4. To work in partnership with staff within the Community Learning Disability Team and other health colleagues to provide a seamless service for people with learning Disabilities and forensic services.

5. To work alongside colleagues to raise the profile of the healthcare needs of people with Learning Disabilities who have a forensic need.

6. To work with the multidisciplinary team to ensure the implementation of effective forensic risk assessment and management strategies designed to effectively manage and mitigate risks.

7. To deliver training for the appropriate development of staff in community services to meet complex clinical needs presented by People with Learning Disabilities and forensic needs in Staffordshire.

8. To implement teaching materials and training packages that enables the development of best practice as agreed and appropriate.

9. To facilitate/support access to forensic services and admission where necessary and appropriate. Advise on suitable reasonable adjustments and support implementation within Learning Disability and Forensic services.

10. To work with colleagues from the Local Authority and/or other health providers to facilitate timely discharge to appropriate community based services, following all locally agreed protocols including CPA and 117 Aftercare entitlements in this regard.

11. To case manage a small number of individuals with complex needs and have a high risk of offending and/or admission to an in-patient setting (identified from Dynamic Risk Register, DSR).

12. To engage in Collaborative Safety Planning (and other interventions) with service users on caseload (where appropriate) with the aim of reducing the risk of offending...

Person Specification

Qualifications & Training

Essential

Qualified RNLD Nurse. Current professional registration or accreditation with appropriate body for qualified role (eg NMC). Relevant forensic qualification/experience.

Experience

Essential

Experience of working with people with a Learning Disability and forensic needs in Community and/or Hospital Settings. Experience of working within multi-disciplinary teams and of being an autonomous practitioner. Experience of managing and prioritising a complex caseload.

Skills, Knowledge & Abilities

Essential

Knowledge of Learning Disabilities, Physical Health. Positive Behaviour Support and Applied Behaviour Analysis, Forensic Practice. Knowledge of current DOLS, Mental Capacity and Mental Health Act legislation.
