Job summary

We are seeking to recruit aNeurodevelopmental Pathway Co-ordinator into a brand-new role to work as anintegral part of the neurodevelopmental pathway (NDP) multi-agency team basedin Halton Child Development Centre covering the footprint of the Borough ofHalton and some areas of Cheshire West.

Main duties of the NDP co-ordinator roleinclude providing specialist support to colleagues on neurodevelopmental issues,taking a key role in liaising with partner agencies to achieve positiveoutcomes for children/adolescents with both a neurodevelopmental difficulty anda mental health condition and taking a key role in feeding back to young peopleand their families the outcome of their multi agency assessment.

The successful candidate would also be expectedto develop the service and the role in line with national and localinitiatives. The post holder will demonstrate an attitude which respects andvalues children, young people and their parents/carers and upholds theBridgewater Trust values.

To work as an integral part of the Neurodevelopmental pathway multi agency team To lead the development and implementation of a diagnostic pathway for children with neurodevelopmental difficulties To provide specialist support to colleagues on neurodevelopmental issues. To take a key role in liaising with partner agencies to achieve positive outcomes for children/adolescents with both a neurodevelopmental difficulty and a mental health condition. To take a key role in feeding back to young people and their families the outcome of their multi agency assessment. To develop the service and the role in line with national and local initiatives.

Duties and Responsibilities

1. Clinical

Complete complex assessments of childrens abilities, development and areas of specific impairment, such as in-depth assessments of behaviour and relationship difficulties.

Screening of new referrals to the neurodevelopment pathway.

Produce a collective clinical formulation and intervention plan where appropriate founded on evidence-based practice in co-operation with the client and their carers.

Actively form and maintain therapeutic alliances underpinned by specialist knowledge and theory; this will also involve co-ordination of other members of the multi-agency team.

Evaluate the effectiveness of the existing pathway and interventions using a range of interpersonal skills, supervision, and standard evaluation measures.

Identify and respond appropriately to Child Protection issues, as described within the guidelines of Bridgewater Community Healthcare NHS Foundation Trust and local safeguarding policies.

Maintain clear and comprehensive clinical notes, to ensure information is accurate and up to date for future reference, in line with ECC Guidelines and complies with the Data Protection Act.

Maintain confidentiality and professional standards at all times.

Identify supervision needs and attend clinical supervision as appropriate.

Organise own caseload and work schedules.

Keep abreast of new clinical developments for young people with neurodevelopment difficulties.

2. Communication

Attend Child Protection Case Conferences as required as a representative of the Trust.

Provide and receive highly complex, sensitive, or contentious information to children and families, and to other professionals within child services.

Communicate effectively with young people and their families, when in crisis and in challenging family situations.

Organise and facilitate multi-professional/agency meetings and reviews.

Elicit highly sensitive information using engagement/communication skills, which are appropriate to the young persons stage of development.

Work as part of an integrated multi professional/agency team, ensuring good communication and working relationships with other professionals in hospitals and the community.

Compile and maintain effective client records in accordance with professional body and Trust standards.

Provide written reports detailing outcome of assessment and recommendations for future support.

3. Service Development

Review existing resource provision

Develop services for children and young people with a neurodevelopmental disorder and mental health needs in collaboration with CAMHS and partner agencies.

Development of neurodevelopment care pathways with partner agencies.

4. Education / Development / Training

Maintain specialist neurodevelopment role within the multi-disciplinary/agency team and provide support and consultation to co-workers within the network.

Provide training to other professionals to improve neurodevelopment health knowledge and skills in line with the Childrens National Service Framework.

Provide specific training and/or presentations to other agencies, statutory and voluntary, and promote and deliver service developments.

Provide training placements for students (of same discipline) as appropriate in agreement with the team and Head of Service.

Contribute to the supervision and training of other professionals within the service as appropriate.

Provide evidence to support development around the Trust appraisal system which includes KSF.

Participate in regular supervision, engage in reflective practice, and be committed to continued development.

Develop own skills and knowledge and contribute to the development of others.

5. Professional Development

To participate in individual performance appraisal and personal development plans, with the manager.

Maintain registration in accordance with their professional body.

6. Shared Team Roles and Responsibilities

Work closely with families, carers and other agencies involved in meeting the needs of the clients.

Ensure maintenance of accurate statistical records as required by the Trust.

Adhere to current research and legislation and work within Trust policies and procedural guidelines.

Participate in orientation, induction, training and education of new staff and students on placement, and staff from other agencies.

Record outcome measure in conjunction with clients and in accordance with professional and Trust policies and procedures.

Participate in the teams routine administrative tasks.

Provide formal documents, such as letters and reports, to other professionals, agencies, and families.

Provide evidence to support development around the Trust Appraisal system which may include the KSF or other competency profiles.

7. Clinical Governance and Research

Maintain personal skills and knowledge through relevant workshops, lectures, journals, and peer supervision.

Takes responsibility for research, evaluation, and audit in response to the clinical and professional interest of the post, and to monitor standards, quality and consumer reaction to the provision of services.

Provides advice and supervision to trainees regarding research skills and competency.

8. Research & Audit

Undertake and participate in appropriate audit, quality assurance programmes and research as directed.

Person Specification

Qualifications

Essential

Masters level qualification or equivalent experience Up to date Professional Registration (to be maintained) Registered Childrens Nurse / Mental Health Nurse / RNLD (Professional Registration with NMC) OR Registered Occupational Therapist, Physiotherapist or Speech and Language Therapist.

Specialist qualifications pertaining to Neurodevelopmental disorder
