Job summary

The Rotherham Foundation trust are seekingmotivated applicants to work at coretraining level (Junior Clinical Fellows)as part of the general surgical team.

These posts would be ideal for a candidate thathas completed foundation training and wishes to begin a career in surgery orhas completed core training and wishes to consolidate their clinical andoperative experience. The breadth of experience available would meanpost-holders would be well placed to complete the MRCS.

Main duties of the job

The on call frequency iscurrently 1 in 10 covering both General Surgery (and Urology out of hours).There is a Surgical Admissions Unit available for acute admissions and threededicated surgical wards. There is resident middle grade cover 24hrs /day andconsultant services are provided by 11 Consultant Surgeons in General Surgeryand five in Urology.

It is a busy department providing a full rangeof elective and non-elective care across breast, upper and lower GI and generalsurgery with an emphasis on laparoscopic surgery. The successful candidateswould gain exposure to surgical conditions in clinics, operating theatre and onthe wards.

The surgical division in Rotherham is afriendly and welcoming environment in which to learn and has a successful trackrecord in supporting its non-training grade doctors who subsequently apply forNational Training posts.

Job description

Job responsibilities

There are12 Consultant Surgeons together with Specialist Registrars, Core SurgicalTrainees and Foundation Trainees. The department provides a wide range ofin-patient and day case procedures and provides emergency and elective generalsurgery services as well as subspecialty services in Colorectal, Breast andUpper GI. The department has an excellent reputation for laparoscopic surgeryand performs significant numbers of laparoscopic colorectal resections, groinand ventral hernia repairs and fundoplications each year. The laparoscopicapproach is routine for appendectomy and cholecystectomy (both elective andemergency). The breast unit perform over 250 cancer resections per year withboth immediate and delayed reconstructions. Complex abdominal wall reconstructionsare undertaken regularly for complex/giant incisional hernia.

Acute Surgical Unit

In 2019the Trust opened a new Acute Surgical Unit, with bespoke assessment facilitiesaimed at maximising our ambulatory care pathways for surgical patients. Asenior surgical assessor reviews patients on presentation and integralultrasound assessment speeds diagnosis. It has reduced non-elective admissionsand reduced length of stay. Initially aimed at General Surgery patients, workis underway to broaden the approach to other surgical specialities who couldtake advantage of the facilities and ambulatory pathways.

UROLOGY

Clinical Lead: Mr Ali Fani

Thedepartment has 5 Consultants (including Trust Locum Consultants and one agencyLocum) plus Specialty and Foundation Trainees. Activity includes outpatient,day case surgery, inpatient elective procedures and emergency admissions

ON-CALL

The rotais compliant with the new contract and is a 1 in 9 full shift rota. Postholders will be expected to co-operate in the introduction of new ways ofworking and monitoring processes with the support of the junior medicaldoctors. Patients requiring major vascular procedures are transferred to theSheffield Vascular Institute, which also provides emergency vascular cover.

The ST3+and equivalents cover urology admissions out of hours and are expected toliaise with the consultant urologist who is on call 24/7 if there areurological problems. There is a daily (including weekends) consultant-ledurology ward round.

EDUCATIONAND TRAINING

The successfulcandidates will have access to all resources currently available in the Trust.This will involve Library and Knowledge Services, e-learning resources,Clinical Skills Training programmes, simulation training and otheropportunities.

Induction will beprovided covering both generic and specialty specific requirements. For newstarters in the UK there will be a one-week programme in PGME prior to startingclinical duties to prepare the post holder for practice in the UK. This may beextended according to individual needs. Initial work in clinical areas would besupernumerary until the Clinical Supervisor agrees the doctor is ready to do oncall.

The post holders willbe expected to develop a portfolio showing on-going professional developmentand can access support from PGME to do this. The post holders will have a namedEducational Supervisor, with whom they will meet on a regular basis forappraisal. They will agree to a Personal Development Plan and receiveassessment in the workplace and regular feedback, including multisourcefeedback.

Each clinicaldepartment will provide access to in-house teaching equivalent to traininggrade doctors. It is intended that a monthly teaching session for doctors onthis rotation will be established.

Funding for externalstudy leave may be available from the department equivalent in value to careergrade staff employed by the Trust.
