Key Responsibilities:

  • Delivering person-centered care and support to individuals with learning disabilities, ensuring their physical, emotional, and social needs are met.
  • Assisting individuals with daily living activities, including personal care, meal preparation, and medication administration.
  • Promoting independence and empowering individuals to develop essential life skills.
  • Building strong relationships with service users, their families, and other healthcare professionals to ensure a holistic approach to care.
  • Supporting individuals in accessing educational, vocational, and recreational activities within the community.
  • Maintaining accurate and up-to-date records of care plans and progress reports.
