Job summary

Thepost-holder will manage a caseload and deal with a wide range of health needsin a primary care setting, ensuring the highest standards of care for all registeredand temporary patients.

Main duties of the job

In accordance with thePractice timetable, as agreed, the post-holder will make him/her-self availableto undertake a variety of duties including surgery consultations, telephoneconsultations and queries, visiting patients at home, checking and signingrepeat prescriptions and dealing with queries, paperwork, reports andcorrespondence in a timely fashion.

Making professional,autonomous decisions in relation to presenting problems, whether self-referredor referred from other healthcare workers within the organisation.

Assessing the healthcareneeds of patients with undifferentiated and undiagnosed problems.

Screening patients fordisease risk factors and early signs of illness.

In consultation withpatients and in line with current Practice disease management protocols,developing care plans for health.

Providing counselling andhealth education.

Admitting or dischargingpatients to and from the caseload and referring to other care providers asappropriate.

Recording clear andcontemporaneous consultation notes to agreed standards.

Collecting data for auditpurposes and complying with QOF requirements.

Delivering enhanced services.

Compiling and issuingcomputer-generated acute and repeat prescriptions (avoid hand-writtenprescriptions whenever possible).

Prescribing in accordancewith the Practice prescribing formulary (or generically) whenever this isclinically appropriate.

MainDuties & Responsibilities:

ClinicalResponsibilities

In general the post-holderwill be expected to undertake all the normal duties and responsibilitiesassociated with a GP working within primary care.
