Are you a qualified and experienced registered pharmacist looking for an exciting new challenge? We have an opportunity for an ambitious and forward thinking pharmacist to join the Care Coordination Community Health Services Team as an integral member of the Essex Partnership University Trust (EPUT) Pharmacy Service.

We are seeking an experienced, enthusiastic, self-motivated pharmacist with excellent communication skills and a clinical background to work within South East Essex integrated health services

The post involves working alongside a multidisciplinary team of healthcare professionals to provide an integrated healthcare service to the community. You will demonstrate a high level of clinical expertise and participate in delivering medicines reconciliation, medication reviews, audits, and other medicines optimisation initiatives.

You will be expected to work independently and show initiative, reporting to the Lead Pharmacist, Community Health Services (South East Essex) and may be required to attend meetings representing the Lead Pharmacist. A full time post is available, but If you are also looking for a part-time working in a responsible and challenging post, this could be the role for you. Hours per week are negotiable.

Job responsibilities

KEY RESPONSIBILITIES Clinical Practice - Conduct clinical medication reviews for patients identified as requiring support- Make appropriate prescribing recommendations and communicate decisions to all stakeholders- Ensure safe and appropriate use of medicines in order to maximise benefits and minimise risk to patients. This will include regular review of prescriptions, identification of drug interactions and adverse effects, advice on clinical monitoring, compliance with legislation (for example, Medicines Act, Mental Health Act), compliance with Trust policies and guidelines.- Provide a clinical pharmacy service to Care Coordination team and assigned wards, and to the multidisciplinary team caring for patients.

- Take an active role in the pharmacy and medicines management team and provide support to less experienced staff providing a service to the Trust, acting as a clinical role model.- Seek advice from senior pharmacy colleagues in order to provide advice to clinicians on unlicensed medicines use and areas of practice where the evidence base is poor.- To work as part of a multidisciplinary team to provide safe, clinically effective and cost efficient use of medicines in patients, including participation in consultant ward rounds, and multidisciplinary team meetings. Provide highly specific advice to clinical team members on the management of complex medicines related issues, such as complex drug interactions, medicines use in pregnancy, unlicensed use of medicines etc.- Make appropriate referrals to the Lead Pharmacist SEECHS and other members of the multidisciplinary team and within the pharmacy team.- Advise medical, dental and nursing staff, and allied healthcare professionals on the actions, properties and the safe, cost effective use of drugs, and be able to critically appraise the literature and give accurate interpretation in order to improve use of medicines. Utilise medicines information resources to further clinical pharmacy practice in community health services- Provide advice which is accurate, evaluated, impartial and up-to-date to ensure the safe, clinical effective and cost efficient use of medicines.- To work as an independent non-medical prescriber, within the designated areas of practice (following training and a period of actively prescribing as a supplementary prescriber if required), to work Responsible and accountable for the assessment of patients with undiagnosed conditions and for decisions about the clinical management required, including prescribing

Community pharmacist applicants are welcome, prior hospital experience not required.
