Job summary

An exciting permanent opportunity has arisen to join Central Cheshire Integrated Care Partnership (CCICP) as a Therapy assistant in the Community Pain Management Service.

The philosophy of the Community Pain Management service is to provide the best options of care, ensuring patients are seen by the most appropriate healthcare professional, at the appropriate time and in the appropriate place, whilst optimising the best health outcome for the patient. This will be managed through effective collaborative clinical triage and delivery of the management of chronic pain through a comprehensive bio-psychosocial model. The service promotes long-term self-management and less dependency on wider health care services such as A&E, GPs, secondary care, ambulance services and mainstream physiotherapy.

The role involves assisting staff and supporting patients with complex pain who are referred to the Chronic Pain Clinic. The Therapy assistant would also assist in the delivery of educational sessions provided by the advanced practitioners in chronic pain, across several sites.

Main duties of the job

Band 4 Therapy Assistant will take responsibility for the management of a defined workload of patients within their Care Community following triage by the qualified therapist.

Work with in their competency and seek qualified therapist guidance were appropriate.

To take a lead in organising therapeutic group activities with patients, such as sleep hygiene, relaxation and mindfulness.

We offer

Full induction on commencement of the role. Supervision from senior physiotherapist and Occupational Therapist. Setting individual objectives through appraisals and competencies will be provided. All generic therapy assistants benefit from regular supervision sessions which are fortnightly to start. Given equal opportunities to undertake a variety of Continuing Professional development (CPD) and Band 4 competencies to develop and support you in your role.

Job responsibilities

SERVICE DELIVERY

To make therapeutic contact with newly referred patients as identified by the qualified therapists following admission and triage. To independently complete specified holistic assessments, interventions and create rehabilitation programmes either independently or with help of a qualified clinician for community-based patients To follow departmental protocols and guidelines referring to the qualified clinician who is available for advice. To hold a caseload as delegated by the qualified clinician. Reporting back at agreed timescales, highlighting any necessary actions. Working independently and supporting the band 3 Therapy Assistants as required. To work with an individual in a variety of settings, such as their home, 24 hours care facility ensuring patient safety and dignity is always maintained. To modify interventions and programmes within defined parameters and within their scope of practice Complete initial assessments and commission a range of basic equipment and adaptations as appropriate to the service user. To complete necessary risk assessments before prescribing basic equipment and review them on a timely basis. To be responsible for the ordering and stock control of materials and equipment as required for the unit. Instruct individuals, carers and other professional in the safe use and maintenance of equipment- simple aids such as walking frames, sticks, elbow crutches and functional aids. To refer patients to appropriate services and health care professionals where patients are found to have a complex health need. Discharge patients within your workload after consultation with qualified member of the team. Attend multidisciplinary meetings and participate in clinical decision-making process for individual patients as delegated by the qualified staff. To ensure accurate, up to date clinical records are kept, always ensuring their security and confidentiality Work within their competency and seek qualified therapist guidance were appropriate. To participate (pro - rota) in 8 am to 8 pm service, 7 days a week to support Urgent Care Response service.

To produce and maintain a personal portfolio to demonstrate on-going personal development. To participate in the Trusts appraisal system, undertaking any identified training and development relating to the post to maintain skills and competencies. To gain appropriate support from senior colleagues and participate in supervision/peer review to enhance clinical practice. To fulfil all mandatory training requirements of the Trust. To promote the safety, wellbeing and interests of patients, staff and visitors to the department. Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care

To demonstrate appropriate time management and organisation skills. To contribute ideas to enable the development and implementation of service improvements. To contribute to the delivery of service objectives and designated projects. To contribute to relevant departmental meetings. Promote a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns. To provide peer support band 3 therapy assistants.

EDUCATION/LEARNING

To contribute to the delivery of formal and informal training to band 3 staff, patients and carers. To assist in the preparation of training materials and organisation of group sessions as required. To contribute to the delivery of designated induction and training programmes to meet the requirements of student placements and newly appointed staff. To contribute to health promotion activities Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation inappraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.
