Job summary

Hospice at Home an independent charitable organisation. We are looking to recruit an experienced registered professional to assist in themanagement of Hospice at Home's clinical services. This includes our nursing,occupational therapy and complementary therapy services. The post holder willbe able to work flexibly to meet the needs of the organisation.

Main duties of the job

The post holder will lead on theassessment and re-assessment of patients and carers receiving home nursing andvolunteer support from Hospice at Home Carlisle and North Lakeland.

The post holder will have experienceof supervising staff in a clinical setting and be required to deputise for the Headof Clinical Services as required.

The post will also requirethe individual to communicate with a variety of Health and Social CareProfessionals, patients/carers and members of the general public on a dailybasis.

We offer day and night nursing care and support, respite provision,specialist services of Lymphoedema and Occupational Therapy, Bereavement andFamily Support, Complementary Therapy and a Befriending Service.

Care is provided by Registered Nurses, NursingAssociates, Assistant Practitioners. Health Care Assistants, Volunteers,Counsellors and Allied Health Care Professionals supported by administrative staff.

2. JOB SUMMARY

To work in a specialised Hospice at Home team as a Registered Nurse, without direct supervision, to lead on the assessment and re-assessment of patients and carers receiving home nursing and volunteer support from Hospice at Home Carlisle and North Lakeland.

To coordinate and manage the Hospice at Home Day Night Nursing Team, acting as Line Manager and co-ordinating Day Night Nursing Service in the absence of the Head of Clinical Services.

Assist the Head of Clinical Services to develop and support nurses and the wider team through an effective appraisal and development review process, identifying learning needs and provide training opportunities.

Liaise with the Head of Clinical Services as appropriate to review care provision.

Participate in direct provision of patient care and liaise with members of the multi-disciplinary team and palliative care team in order to achieve this.

3. ROLE OF ORGANISATION

To provide an expert and comprehensive Hospice service to enhance the experience of patients with cancer from diagnosis through treatments to palliative and end of life care and to others with progressive life threatening conditions. The service also supports relatives and carers providing care predominately in the home or within the local community.

Hospice at Home Carlisle and North Lakeland is a charitable organisation working alongside and in collaboration with other specialist palliative care providers, local primary care services and adult social care as well as acute settings and residential care.

The service provides nursing care and support, respite, specialist services of lymphoedema and occupational therapy, family and bereavement support, counselling and complementary therapy.

The service utilises Registered Nurses, Nursing Associates, and Assistant Practitioners. Health Care Assistants, Volunteers, Counsellors and Allied Health Care Professionals supported by administrative staff.

4. DUTIES AND RESPONSIBILITIES OF THE POST

Responsibilities for Patients

Using a holistic palliative approach, assess, plan and implement individualised patient care plans. Regularly re-assess care and support needs of both patient and family and support them during their illness, being perceptive to their changing physical and emotional/health needs. On a day to day basis prioritise care arrangements for patients receiving home nursing, deploying staff appropriately and informing relatives as to why changes have been made. To have a problem solving approach both internally and liaising externally to the wider MDT. Provide direct clinical nursing care when required. Undertake and regularly review risk assessments for patients and carers being supported by the Hospice at Home nursing and support team. Be able to use own judgement to assess, plan and prioritise workload. Liaise with the wider multi-disciplinary team about patients, ongoing care needs and provide reports where necessary for patients being considered for continuing care. Be frequently exposed to upsetting emotional situations. Undertake lone working in environments that may expose the service to abusive, violent or challenging behaviours. To record data/information accurately and clearly in nursing/computer records, maintaining patient confidentiality and data protection. Document patient records following visits made to patients. Report identified safeguarding concerns appropriately using guidelines and procedures.

Responsibility for Families and Carers

Monitor standards of care being provided to patients by the Hospice at Home clinical staff. Attend referral/discharge meetings when appropriate. Attend, participate or facilitate de-briefs and case conferences for patients their relatives and carers. To be aware of young families and children within care environment and refer on to appropriate service as required.

Responsibility for Administration

Monitor staff attendance for mandatory training. Ensure that the Hospice at Home team uses resources effectively and in sustainable and environmentally friendly manner. Identify problems with resources and resource use promptly and recommend corrective action. Responsible for authorising staff annual leave. Responsible for initially checking staff time sheets on a monthly basis. To attend designated meetings in the absence of the Head of Clinical Services. To deliver talks and presentations to the general public, trustees and others as appropriate.

Responsibility for Self-Development

Demonstrate clear understanding and awareness of own responsibilities under the NMC Code, working within the parameters set, linking own practice to The Code at all times. Participate in regular supportive conversations. Take a proactive approach to own annual appraisal and development needs. Attend study days in line with personal and professional development plans. To keep updated with current policies and procedures. To keep updated with current nursing developments. Promote high professional standards of care management by maintaining and developing knowledge of current clinical practice using the best available evidence base and to support less experienced staff to do likewise.

Responsibility for Service Development

Provide support and supervision on a one-to-one basis and when need arises to all Hospice at Home clinical staff. Plan and lead monthly team meetings in association with the head of clinical services. Contribute to the annual appraisal system and ongoing support to achieve set objectives. Challenge those whose views and actions are not consistent with developing knowledge, ideas and work practice. Review and develop nursing documentation used by the Hospice at Home clinical staff. Undertake service audit and review as an integral part of the role. Contribute to policy and practice changes arising from audits and complaints. Assist in the introduction of new service initiatives. Assist in the recruitment and selection of nurses for the Day/Night Nursing team. Plan and contribute to the induction programme for Day/Night Nursing and other services. Member of the on-call (telephone only rota. Involved in planning and adjusting care for patients currently receiving Hospice at Home nursing service and supporting staff in difficult situations (includes weekends and some bank holidays). Updating appropriate health and social care professionals.

Responsibility for Training

Plan, prepare and deliver where appropriate mandatory training for Hospice at Home staff and volunteers appropriate to individual requirements, in conjunction with other key workers. Create and sustain a culture which promotes self-development. Assist the Nurse Team Lead to deliver study days covering a range of subjects and speakers. Be a resource/mentor for students educating them during their placements.

Clinical Governance

Maintain accurate records and documentation which comply with professional, legal and administrative requirements. Support the Care Quality Commission regulation process, providing evidence of quality standards and care. Respond appropriately to incidents and near miss situations.

7. WORK SETTING AND REVIEW

To work unsupervised on a daily basis, manage and prioritise own workload. Work is supervised rather than managed. Reporting to line manager on a monthly basis and undertaking any other reasonable duties that may be required. The post holder will have responsibility for their own wellbeing which will be supported by the organisation.

Experience

Essential

Proven post registration experience and experience of working effectively in a team. Working with palliative care patients and families. Experience in Appraisal process and the training, support and development of staff. Providing clinical supervision to others. Ability to use initiative and prioritise effectively. Ability to communicate sensitively and diplomatically with a wide range of people.

Desirable

Community working. Experience of Line manager others
