Main duties of the job

The post holder will work as part of a team to provide a safe, effective, efficient care environment for patients, relatives and carers. They will beresponsible for assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards.

To assist the management of patient flow in partnership with the ward management structure, and assist in monitoring the performance.

The post holder is required to work within the NMC code of practice and professional guidelines.

To work within the nursing and multi-professional team to ensure a high standard of care to a defined patient group, competently performing the necessary technical/invasive/physical aspects of care for this defined patient group.

Job description

Job responsibilities

Assess, plan, implement and evaluate programmes of care with reference to patients, without direct supervision. Co-ordinate the provision of seamless care for patients from admission/transfer to discharge by utilising efficient verbal and written communication practices with all members of the Multi-Disciplinary Team/Primary care providers.

Requires to take responsibility for a defined group of patients within the clinical speciality.

Provide teaching, guidance, supervision and support for junior staff, unqualified staff and students where required.

Ensure that all written documentation within the ward/departments is clear, concise, timely and complies with the NMC's standards for records and record keeping. Report all accident/incident/complaints involving patients, carers and staff to the senior nurse and commence enquires or documentation as necessary.

Carry out clinical duties in accordance with National, Trust and local standards and policies.

Take responsibility for the correct use and operation of medical equipment in accordance with relevant trust and national guidance.

Participate in ward/department and directorate audit activity where appropriate and assist in the development of evidence based practice as required.

Be aware of, observe and adheres to hospital, organisational and national policies.

Be responsible for the safe transfer of medically ill patients with a variety of multiple complex illnesses and highly dependent.

Use monitoring, knowledge, experience and discretion when administering medications to patient to allow safe withholding/stopping of drugs.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.
