The post is suitable for a patient-focused and team player technician who is sensitive to the needs of the patients and carers. You would require excellent communication and interpersonal skills. The role is embedded in the ward teams by supporting our nurses and pharmacists to resolve any medicines management issues.

Main duties of the job

You will be responsible for providing a safe, efficient and accurate ward-based pharmacy service for hospital inpatients by liaising closely with the pharmacy team, nurses and medical colleagues. This role will help you to develop medicines optimisation, patient counselling skills and ensuring safety and efficiency during the patient's discharge process.

You will also be required to support the Inpatients Pharmacy as an ACPT two hours a week. Therefore, a systematic and flexible approach to working is an essential requirement.

Job description

Job responsibilities

To provide a pharmaceutical Medicines Management service to patients. This includes:

Communicating detailed information to patients & carers about drug doses, indications, side effects and efficacy.

Liaising widely ( GP staff, Nursing homes, patients, carers etc) to ensure the timely availability of patients medication.

Assessing Patients Own Drugs for suitability for use in the Trust.

Record accurate written medication reconciliations for newly admitted and transferred patients.

The dispensing and final checking of clinically screened discharge medicines against the TTO before release to the patient.

Provide a safe and efficient supply of pharmaceutical products to patients. This may involve the:

Dispensing and final checking of drug products and compliance aids after screening by Pharmacist.

Working in the dispensary on a session basis, ensuring that good dispensing practices are followed and that all staff are working within the appropriate Standard Operating Procedures.

Liaise with clinical pharmacists to highlight and refer clinical problems, ensure that requests are processed safely and prioritised to match clinical urgency.

Participate and provide an Omnicell service and cycle counts as per Directorate.

Supervise Rotational Technicians, Pharmacy Assistants, Pre-registration Pharmacy Technicians and Pre-registration Pharmacists to ensure that work is conducted to a high standard and in accordance with Standard Operating Procedures.

Participates and deliver Medicines Management education, training, and assessment of staff within the Directorate.

Undertake the assessments and appraisals of rotational pharmacy technicians.

Participate in the education and training of non-pharmacy staff ( nurses) as required.

Routinely gather data for the audit of services and assists with analysis.

Person Specification

Knowledge/Qualifications

Essential

BTEC Pharmaceutical Science/NVQ Level 3 or GPhC accredited equivalent Accuracy Checking Pharmacy Technician (ACPT) qualification LPET Technician Self Development Programme- Stage 2, AIMS, MOP from HEE LaSE or equivalent Medicine Management qualification Registration as a Pharmacy Technician with the General Pharmaceutical Council (GPhC) Mandatory professional CPD as defined by General Pharmaceutical Council.

Desirable

A1 workplace Assessor or working towards Practice Supervisor, Train the trainer or equivalent qualification

Experience

Essential

Have worked in a hospital pharmacy dispensary Medicines management experience in a ward based setting, medication history taking, POD assessment etc. Patient consultation experience Experience of pharmacy stock distribution and top-up services Previous experience of supervising staff and workload on a day-to-day basis Previous experience of delivering induction and competency based training

Desirable

Experience of education and training of pharmacy staff Experience of education and training of MDT staff Recruitment and selection experience Demonstrated experience of appraising staff and setting performance objectives Previous evaluated experience of participation in project and audit work

Skills

Essential

Demonstrated ability to counsel and give advice to patients on their medications. Demonstrated ability to accurately record patient medicine reconciliation.
