Job summary

Resettlement Assistant hours per week

Standard working days Monday, Tuesday and Wednesday (with flexibility required).

We are looking to recruit a colleague who will provide secretarial and administrative support to the Resettlement Team (part of the Midlands Centre for Spinal Injuries) to assist with the discharge process to prevent delays.

The role involves working closely with the Resettlement Officers throughout all stages of the discharge process and provides a key role assisting patients and their families to complete application forms and follow up progress with the relevant agencies.

You will join a team of 3 Resettlement Officers, a Resettlement Discharge Assistant, 2 Generic Support Workers and a Resettlement Assistant.

Main duties of the job

Working within the Resettlement Team involves liaising with external bodies such as Social Services and CCGs when setting up new patient care cases, following up referrals and liaison with community teams, and family members .

Job responsibilities

follow up referral to Social Services to ensure allocation of named Social Worker prior to Discharge Planning Meeting. To notify the community team (GP, DN, SW, COT) in writing, of patient admission and provisional discharge date, on behalf of the Consultant. To notify the Community Team of Discharge Planning Meetings To assist family members with applications for eligible travel expenses and accommodation. To contact charitable Agencies to assist with above To work closely and effectively with the Resettlement Team in order to support communication throughout the service, take messages and relay information. On instruction of to liaise with Housing Departments and assist patient to complete housing applications. To be responsible for liaising with the Department of Works and Pensions representative and organising the Benefit Clinic every Friday and collating MDT reports and other relevant documentation. To manage own time effectively to meet service objectives, prioritising work according to the demands of the service. To maintain effective communication To enter out- patient and community activity onto PAS on behalf of the Resettlement Officers. To update patient information EDDs onto Lorenzo/PSAG. Establish and maintain effective exchange of patient information between Midland Centre for Spinal Injuries (MCSI), and community colleagues maintaining confidentiality. To protect all confidential information concerning patients, only making disclosures with the patient consent. To discuss in a sensitive manner information which may cause emotional distress. To maintain clear and accurate records in patients resettlement notes of the service activity in order to monitor working practices. Secretarial skills, RSA 3 or NVQ 3 Business Administration ECDL or equivalent qualification/experience
