What you will be doing
:As a qualified Practice Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments.
You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented.
As a Practice Nurse, you will be:
Adhering to the NMC Code of Conduct and facilitate the compliance with local, Trust and national standards of practice. Developing and supporting physical health interventions used in the clinical area using knowledge of current best evidence-based practice. Engaging actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice. Working collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments. Monitoring and maintaining Physical Health Dashboards.Things that you will have:

Join the multidisciplinary team at Three Valleys Hospital, care for people with complex mental health issues in a safe and supportive environment.
There are a range of services across five wards – including a specialist dementia service - where you’ll gain experience working with gender-specific distinct rehabilitation and step-down services. All 43 bedrooms have en-suite facilities and the communal areas are spacious with a warm, homely feel.
