Job summary

We are excited to be recruiting an additional PCN Health & Wellbeing Coach, to further expand our growing Personalised Care Team.

You will be working with patients that accept that they need to make health and lifestyle changes but do not feel ready to make the changes, to coach and support them to a point where they are ready to make changes and then link them in with a member of our Social Prescribing team to enable them to make the changes.

We also run regular 'Walk and Talk' sessions for our patients that provide an opportunity for groups of patients to go for a supported walk and then meet at a local cafe for a drink and an opportunity to engage with other members of the group.

These sessions are also supported by members of the Sky Blues in the Community Team who work with us to provide additional Health and Wellbeing Coaches that run Healthy Lifestyles programmes for our patients.

Main duties of the job

You will be a member of our Personalised Care Team that includes Social Prescribers, Health and Wellbeing Coaches, Care Coordinators and Mental Health professionals to support patients registered at our member practices to make positive changes to improve their health and wellbeing.

You will also link in with other members of the PCN workforce and GP surgery teams to provide joined up care.

The role will require you to work with patients that have been referred to you by colleagues to support them to understand that they need to make changes and help them to reach a point where they are ready to make a change. These patients are then passed on to one of our Social Prescribers who work with them to identify the most appropriate options and link them into relevant local services.

You need to be able to work on your own initiative to manage your own caseload, but must also be able to work as part of a team to ensure a streamlined experience for our patients.

About us

We are a large Primary Care Network (PCN) with 16 member practices, representing over 120,000 patients.

We have an extensive Additional Roles workforce that includes Dietitians, First Contact Physiotherapists, Pharmacists, Physician Associates and Nurse Associates, as well as the Personalised Care Team.

You will be one of twenty members of the Personalised Care Team that is overseen by a Senior Social Prescriber and a Senior Care Coordinator who have a remit to explore opportunities to grow and develop our services, as well as provide leadership and support to their team members.

Job Purpose/Summary

To act within professional boundaries, working alongside a team ofclinicians, social prescribers, health & wellbeing coaches, administrators andfacilitators in general practice within the Nuneaton & Bedworth PrimaryCare Network (PCN). You will improvepatients health outcomes and the efficiency of the Primary Care team byproviding coaching to those who are not yet ready to fully benefit from socialprescribing. Offering personalisedsupport to patients/carers and families to support them to actively participatein their own healthcare. Through face-to-facecontact or virtually by both group and individual circumstances.

The postholder will be an integral part of the PCN team, workingin allocated practices across the Primary Care Network, as well as being a valuedmember of the Personalised Care team.

Key Responsibilities

1.Manage and prioritise a caseload, in accordance with the healthand wellbeing needs of the population through taking an approach that isnon-judgemental, based on strong communication and negotiation skills, whileconsidering the whole person when addressing existing issues. Where medical treatment needs review ormodifying, the Health & Wellbeing Coach will refer people back to otherhealth professionals within the PCN or practice.

2.Utilise existing IT and MDT (Multi-Disciplinary Teams) channels toscreen patients, with an aim to identify those that would benefit most fromhealth coaching.

3.Providepersonalised support to individuals, their families and carers to support them tobe active participants in their own healthcare; empowering them to manage theirown health and wellbeing and live independently.

4.Coaching and motivating patients through multiple sessions toidentify their needs, set goals and supporting patients to achieve theirpersonalised health and care plan objectives.

5.Providing interventions such as self-management education and peersupport.

6.Supporting patients to establish and attain goals that areimportant to the patient.

7.Supporting personal choice and positive risk taking while ensuringthat patients understand accountability of their own actions and decisions,thus encouraging the proactive prevention of further illnesses.

8.Organising,running and facilitating small group sessions, including coordinating inputfrom health care professionals. Thiscurrently includes regular Walk and Talk sessions in Nuneaton and Bedworth,but includes a requirement to identify future opportunities for new groups.

9.Workingin partnership with the Social Prescribing service to connect patients tocommunity-based activities which support them to take increased control oftheir health and wellbeing.

10. Increasing patient motivation toself-manage and adopt healthy behaviours.

11. Work with patients with lower activationscores to understand their level of knowledge, skills and confidence (their Activationlevel), when engaging with their health and wellbeing and subsequentlysupporting them in shared decision-making conversations.

12. Utilise health coaching skills tosupport people with lower levels of activation to develop the knowledge, skillsand confidence to manage their health and wellbeing, whilst increasing theirability to access and utilise community support offers.

13. Support clinicians in facilitating groupconsultations including virtually.

Person Specification

Qualifications

Essential

GCSE Grade C or above in Maths and English, or equivalent qualification

Desirable

NVQ Level 3, Advanced Level or equivalent or working towards and a commitment to professional and personal development

Experience
