Job Summary

We are seeking to recruit a clinical mental health practitioner with a core professional registration (Nursing, Social Work, Occupational Therapy) to work within an exciting new service development within Pennine Care as a clinical senior mental health practitioner working alongside the clinical lead advanced practitioners within the team. We are looking for individuals who are passionate about working with children and young people and working innovatively across pathways and traditional boundaries to ensure that young people get the right care, from the right clinician/team, at the right time and in the right place in order to improve their outcomes and the families/carers experience of mental health support. We are looking for an individual with significant clinical experience, preferably within CAMHS and a specialism in a relevant area/therapy who can provide clinical guidance and support to our mental health practitioners whilst also taking the lead of case management with high level complexity cases.

This is an exciting opportunity for practitioners to work as a Band 7 Mental Health Practitioner offering assessment, support, signposting and brief therapeautic intervention where appropriate to young people who required crisis support within our CYP crisis teams which include Home Treatment, Rapid Response and Enhanced Rapid Response-paediatric liaison within the Pennine Care footprint.

Main duties of the job

o To assume responsibility for management of a complex caseload and exercise the highest standards of autonomous professional responsibility for the assessment, treatment and discharge of children, young people and their families/carers using specialist knowledge and evaluation of care programmes in collaboration with children, young people and their families/carers liaising with and referring to relevant agencies as appropriate.

o To undertake specialist assessment of children, young people with mental health needs referred to the service including risk assessment, gathering appropriate information from their family/ carers and other relevant agencies.

o Following assessment, to develop a formulation and plan for intervention, based on an appropriate conceptual framework in collaboration with the child, young person and parents/carers, communicating this to the referrer and other relevant professionals.

o Communicate highly complex information in a way that supports understanding and collaborative working and breaks down barriers whilst addressing issues of safeguarding and risk.

To have highly developed specialist knowledge, underpinned by theory and practice that allows delivery of evidence-based interventions in line with NICE

In line with the NHS Long Term Plan our CAMHS Care Hub has done extensive work around developing an exciting new Acute Care Pathway for children experiencing a crisis in their mental health. The aim within this pathway is to ensure that at the time of a young person experiencing a crisis our services can respond in the right way and at the right time with the right support to try and keep young people out of hospital and care for them in the community so they are able to experience the least amount of disruption to their lives.

Job responsibilities

Education and Development:

To maintain an up-to-date knowledge of current issues and practice in child and adolescent mental health.

To participate in locally agreed appraisal process and staff development, ensuring this is regularly reviewed and evaluated.

To undertake mandatory training as per Trust policy and participate in other relevant training to meet the needs of the service and as outlined in the individuals development plan

To receive regular clinical and line management supervision from a suitably qualified clinician and, where appropriate, other senior professional colleagues.

To provide teaching and training in a range of settings as required and in line with post holders experience and skills.

Clinical Governance:

To be responsible for own continuing professional development and maintaining own professional registration, and any requirements to maintain registration.

To have current working knowledge of relevant professional body code of practice, standards and guidance, understand implications for and be able to apply to practice.

To maintain up to date knowledge of NICE guidance and evidence-based practice, monitoring and maintaining quality and practice standards within the team.

Have responsibility for the accurate recording and submission of all relevant data and statistical records as required by the Senior Leadership Team

To attend and actively participate in line management supervision with designated professional.

To ensure attendance and participation in safeguarding supervision in line with Trust policy.

Participate in identified quality initiatives to promote the development of the CAMH service.

To communicate in a timely manner any issues concerning service provision, clinical practice and multi-agency working together to relevant line manager/supervisor.

To contribute to audit, research, and evaluation of the service.

To represent a professional viewpoint in relation to nationally accepted good practice and to ensure a high level of ethical standards and professional conduct.

Supporting Transitions

Work with the child or young person to establish more immediate coping strategies.

Work with children and young people to identify the skills, abilities and behaviour and give guidance to children and young people about socially acceptable behaviour and any associated risks of danger, harm and abuse that will enhance their life chances and equip them to be responsible adults and effective citizens.

Ensure that all the relevant agencies are provided with the information they need to help the child or young person and those involved in their care to seamlessly move between tiers of Child and Family Services.

Safeguarding

Appointments to regulated and controlled activities require an enhanced DBS disclosure.

Pennine Care NHS Foundation Trust (PCFT) employees have a statutory duty to promote the welfare of children and young people and to protect adults; enabling them to live free from harm, abuse and neglect.

Engagement with safeguarding training, in line with responsibilities, is mandatory for all PCFT employees. PCFT employees must familiarise themselves with policies and procedures and adhere to these.

It is the responsibility of all staff to report safeguarding concerns and familiarise themselves with who to contact in order to do this or seek further guidance.

Professional and Personal Development

The post holder will be involved in a formal appraisal and yearly conversations review with their manager at least every 12 months. Once performance / training objectives have been set, the staff members progress will be reviewed on a regular basis, so that new objectives can be agreed and set, in order to maintain progress in the service delivery.

The post holder will be expected to take responsibility for their own professional development and will be supported by the Trust to achieve development opportunities as appropriate.

Confidentiality and Information Governance

Confidentiality is of prime importance. In the normal course of duties, the post holder will have access to confidential documents and information relating to patients, service users, staff and contractors, as well as information of a commercially sensitive nature. Such information should not be communicated to anyone outside or inside the NHS unless done in the normal course of carrying out the duties of the post. Disciplinary action will be considered where a breach of confidence has been established.

All information obtained or held during the post-holders period of employment that relates to the business of the Trust and its service users and employees will remain the property of the Trust. Information may be subject to disclosure under legislation at the Trusts discretion and in line with national rules on exemption.

The post holder must maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date. The post holder must only access information, whether paper, electronic or in other media, which is authorised to them as part of their duties.

The post holder must work to the requirements of data protection laws as applicable to the UK, which includes the General Data Protection Regulations (GDPR).

Health and Safety at Work

The post holder is required to take reasonable care of the health and safety of themselves and other persons who may be affected by their acts or omissions at work and to co-operate with the Trust in adhering to statutory and departmental safety regulations.

The post holder is responsible for ensuring that they do not intentionally or recklessly misuse or interfere with anything provided in the interests of health safety or welfare misuse of equipment.

The post holder is required to contribute to the control of risk and must report immediately, using the Trust Incident reporting system, any incident, accident or near miss involving patients, service users, carers, staff, contractors or members of the public.

All Trust sites have been designated a no smoking area. The post holder is therefore advised smoking is not permitted within the hospital premises or grounds or whilst representing the Trust in the course of their duty. While the Trust will not discriminate against employing smokers, all prospective employees should be aware of this policy.

Infection Control

Infection Prevention and Control is the responsibility of all Trust staff.

All staff members have a responsibility to protect service users, visitors and employees against the risk of acquiring health care associated infections by consistently observing Trust Infection Prevention and Control Policies and procedures and best practice guidance in order to maintain high standards of Infection Prevention and Control.

Relevant core professional qualification in Mental Health or social care plus evidence of continued professional development in a CAMHS related area plus experience to masters level Evidence of Professional Registration and continued professional development Demonstrable post qualification, experience, including experience of working with complex mental health problems and working with children and families in Particular Experience of specialist mental health assessment and treatment of children and adolescents and their families/carers where appropriate Substantial experience of working as a senior mental health worker with children and families with in a planning and delivering mental health setting. Presentations and training group. Experience of developing clinical formulations with children and families. Evidence of leadership skills, experience of supervising staff and students, leading projects and managing change. Ability to apply research to practice A successful track record in making improvements in service delivery or operational effectiveness. Currently engaged in an advanced level of practice and able to demonstrate a high degree of professional effectiveness' Experience of hard to reach diverse community group and individuals with remerging complex needs, dual diagnosis, people with additional disabilities and safeguarding Experience of assessing and managing risk including safeguarding Experience of developing and providing evidenced based assessment, formulations and care plans and effective use of ROMS Experience of liaison and working with a range of other relevant agencies and professionals including managing difficult.
