Main duties of the job
The primary focus of the role is work as an autonomous practitioner providing high quality and timely patient centred care. A key role of this post is to have responsibility for those patients attending our Same Day Care bases who have primary and urgent care problems and could be safely and appropriately seen by an autonomous practitioner.
The post holder will be responsible for the assessment, diagnosis, treatment, referral and discharge of a diverse range of patients with minor illnesses and within their level of competence and agreed protocols.

NHUC is anot-for-profit Community Benefit Society, working in partnership with the NHS.NHUC provides services through two divisions, Same Day Care (SDC) and TalkPlus.
SDC providesservices through the Integrated Urgent Care (IUC) contract which covers the inHours Clinical Assessment Service (CAS) and Out of Hours (OOH) provision forNorth Hampshire, North East Hampshire and Farnham (NEHF) and Surrey Heathplaces who are part of the Hampshire and the Isle of Wight ICB and Frimley ICBrespectively. SDC also support projects such as ED streaming in Frimley ParkHospital and the Aldershot Urgent Care Centre in the Aldershot Centre forHealth. It also holds some smaller contacts such as supporting CommunityHospitals OOH and Flu response. Finally SDC has a virtual care arm supportingpatients at home.
TalkPlus arepart of the Improving Access to Talking Therapies programme, providing supportto people with common mental health problems in Northeast Hampshire and Farnhamplace. TalkPlus offers a range of treatments to people who are experiencingsymptoms of stress, anxiety, low mood, depression, panic, phobias or OCD.TalkPlus has care pathways to provide timely treatment for perinatal andmilitary veteran referrals.

Clinical Care
Actas an expert practitioner, demonstrating clinical competence and a knowledgebase supplementing those associated with conventional nursing/ paramedic/pharmacist roles.
Undertakean element of triage, streaming patients to the appropriate service or closingcall with appropriate advice and safety netting.
Undertakeface-to-face consultations with patients as an autonomous practitioner andusing own clinical judgement, diagnose, treat, refer and/or discharge patientswithin competence and home visit if required
Make direct referralsto primary, secondary and social services within locally agreed pathways,guidance and protocols.
Ensurethat patients receive high quality clinical care, delivered in a timely manner.
Bean independent prescriber and safely supply and administer medicines.
Provideclinical advice to Administration staff where required
Ensurecomplete and accurate documentation of each and every patient contact
Workas an autonomous practitioner and as part of multi-disciplinary andmultiagency teams in order to ensure patients needs are met.
