Job summary

Be an active member of the clinical team, providing a comprehensive Physiotherapy service to residents of Brent.

Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.

Be responsible for the supervision of junior staff, rehabilitation assistants and physiotherapy students

Work in a variety of settings including clients own homes, clinics, residential and care homes, community facilities, continuing care homes and community hospitals.

Support self and service development through active participation in Continuing Professional Development (CPD) and by participating in any audit or research being undertaken by the service

Main duties of the job

Undertake specialised uni-professional and multi-professional assessments.

To develop and implement treatment plans for clients referred to the different services. These clients will present with a variety of complex multipathologies within specialist areas, including falls, neurological conditions, musculoskeletal conditions, age related disorders, osteoporosis, learning disability and/or mental health diagnosis.

Participate in the Single Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate.

Be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. To be able to interpret and follow these guidelines in circumstances where clients do not have capacity to provide informed consent.

Have working knowledge of clinical guidelines, specific to the area you are working in and, with the guidance of your supervisor, to translate these guidelines into clinical practice.

Job responsibilities

Be responsible for the safe storage and safe moving and handling of physiotherapy equipment.

Work closely with other members of the multi-professional team Occupational Therapists, Nurses, Dieticians, Podiatrists, Pharmacists, Care Managers and Psychologists to provide a comprehensive, seamless service aiming to improve and maintain functional physical health for clients using the service.

Work as a lone practitioner, within the guidelines of the Lone Working Policy, when performing home visits

Identify clients who need support from outside agencies, using clinical reasoning and analysis skills. To liaise with, and to refer clients to these services which include other Healthcare Services, Healthcare Professionals and Voluntary agencies as appropriate.

Qualifications

Essential

Degree or diploma in physiotherapy Registered with the Health Care Professions Council Hold relevant right to work in the UK documentation.

Desirable

Membership of Chartered Society of Physiotherapy and / or relevant special interest groups

Experience/ Knowledge

Appropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community Appreciation of constraints to working in the community Ability to assess patient conditions, establish the patients problem, formulate and implement a treatment plan Experience of MDT working and understand their roles Experience of supervising juniors/assistant/students Experience in community work Post registration experience at band 6 level in a wide range of clinical areas including elderly rehabilitation, community rehabilitation, falls management, dementia care, respiratory care and orthopaedics Experience in quality improvement and service evaluation.

Previous community experience within rehab Experience of using an electronic clinical records system

Advanced skills in the assessment and treatment of people with complex multi-pathologies Advanced knowledge and understanding of complex medical conditions and their relation to people who fall Knowledge and understanding of National and local frameworks and guidelines for older people. Knowledge of outcome measures and evidence based practice associated with people with physical disabilities Ability to problem solve, prioritise and demonstrate advanced clinical reasoning Advanced communication and report writing skills, in both verbal and written format Skills in supervision and appraisal of assistants, junior therapists and students IT skills - Microsoft Word, Power point, Excel, Outlook Knowledge and understanding of clinical governance An understanding of the importance of involving service users in their treatment and in service development ? Knowledge of project management including planning, implementation and review

Demonstrate physical ability to be able to carry a standard backpack with medical supplies/laptop required to fulfil essential clinical duties.
