Job summary

To have clinical responsibility for assessment and treatment planning for people referred and will be required to work autonomously and as part of the multidisciplinary care team

The clinical caseload will comprise of adult patients with physical and cognitive difficulties within the community

To supervise junior colleagues and assistants

To collaborate with colleagues across own and other community teams, acute teams, GPs, social services and voluntary sector to optimise patients' journey, care, wellbeing and outcomes.

To assist in the overall development and management of physiotherapy and community rehab services, and in the development of integrated pathways

Main duties of the job

To undertake specialist physiotherapy and holistic assessment of patients as an autonomous practitioner, interpret results appropriately, use clinical reasoning skills and knowledge of evidence based practice, in relation to the individual's clinical and psycho-social issues to determine treatment options, recommending the best course of action. To undertake treatment planning and implementation of agreed goal orientated intervention with client/carers. To contribute towards single assessment and trusted assessment processes To provide comprehensive intervention to patients in the community, making use of assessment and treatment facilities where appropriate. To be responsible for a clinical caseload working within multi-disciplinary teams, sharing care appropriately ensuring patients and carers receive optimum level of therapy and holistic care within available resources. To carry out multi-disciplinary screening assessments and discuss outcomes with multidisciplinary team members

Job responsibilities

To abide by the HCPC / CSP rules of professional conduct and local professional and quality standards. To develop and maintain personal, clinical and managerial skills through supervision, CPD and a personal development plan agreed with the line manager/professional manager. To maintain a current knowledge of developments within the NHS and physiotherapy. To participate in clinical governance/audit/clinical effectiveness projects in this clinical area. Education/Qualifications

Essential

Degree or diploma in physiotherapy Registered with the Health Care Professions Council Up to date relevant knowledge through CPD

Desirable

Membership of the Chartered Society of Physiotherapy

Experience

Essential

Relevant post registration experience covering core rotations including community experience and falls, older people, frailty, long term conditions, MSK, Ability to use electronic clinical system RIO, EMIS, Cerner Previous NHS experience

Desirable

Using EMIS clinical system

Skills/Abilities/Knowledge

Essential

Ability to demonstrate on-going CPD and the use of evidence based practice Awareness of current issues in health, including clinical governance Ability to assess, set SMART goals and evaluate
