The successful candidate willhave experience in an acute hospital setting and be able to demonstrate aknowledge of caring for patients with a wide range of medical conditions,including their nursing and medical management. They will possess goodorganisational skills and be able to contribute to the development of flexibleworking arrangements within the team.

Main duties of the job

Youwill work closely with our experienced and professional team to assess,examine, diagnose, treat and discharge emergency medical patients. Performingdiagnostic and prognostic investigations following clinical pathways, forexample, DVT, PE, etc.

They will manageunpredictable patterns of work, accepting referrals from health careprofessionals, triageing and prioritising patient care according to clinicalneed and departmental workload.

They will work closely withMedical and Nursing professionals Allied to Medicine teams in the trust tofacilitate the prompt evidence basedtreatment of patients.

Patients requiring urgentor emergency treatment will be escalated appropriately to members of theclinical team or to other specialities.

Clinical assessment,investigation, management, treatment and follow on will be clearly documentedand communicated with patients and with other health care professionals toensure patients receive streamlined and evidence medical and nursing care.

They will act as clinicalsupervisor and role model to other members of the medical SDEC nursing teamAdvice and guidance will be provided to other health care professionals

Job responsibilities

Performdiagnostic and prognostic investigations following clinical pathways, forexample, DVT, PE, etc.

Work closely with Medicaland Nursing professionals Allied to Medicine teams in the trust to facilitatethe prompt evidence based treatment ofpatients

Triage patientsprioritizing clinical need and highlighting patients who require immediatemedical intervention to the medical team

Act as a resource for allmembers of the multi-disciplinary team, providing advice regarding aspects ofmanaging ambulatory sensitive conditions

Work with medical andnursing colleagues to provide a good standard of holistic care for patients.

To coordinate within theunit, taking referals and lead the staff on duty ensuring patient safety.

Work with Ward Coordinatorsand Patient Flow to provide emergency admissions access to the service

Assist the team to developinnovative and flexible approaches to care.

Ensure accurate and legiblenursing records are kept in accordance with trust policy and NMC

Assisting in developingclinical protocols/guidelines and standards to facilitate practicedevelopments.

To assist the AdvancedNurse Practioner to facilitate the development of the clinical skills of thenursing and medical team, contributing towards the ongoing education anddevelopment needs of the unit.

Support the development ofan integrated approach to the delivery of Acute Medicine and Emergency Care.
