Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to lead our Frailty Team in providing high-quality, patient-centered care to frail patients in our acute medicine and unscheduled care services.
The successful candidate will have expertise in the management of frail patients, supporting admission avoidance and safe, prompt return to the community.
Key responsibilities include:
- Providing clinical leadership to a team of staff within the Frailty Team
- Developing and enhancing knowledge and skills to influence consistent standards of clinical practice
- Communicating effectively with multi-disciplinary teams and external stakeholders
- Assessing, diagnosing, treating, and discharging adult patients with frailty
- Working independently with a high degree of autonomy and taking part in clinical supervision
The ideal candidate will have a strong background in acute nursing, experience working with frail patients, and a keen interest in improving their role and contributing to the development of the Frailty Team.
Requirements
- Masters level or willingness to work towards
- Substantial experience at band 6 in an acute medical setting
- Teaching qualification
- Independent prescribing or willingness to undergo
- Experience of working in an ambulatory emergency care setting
- Substantial specialist experience of frail patients
What We Offer
We offer a competitive salary of £46,148 to £52,809 per annum, pro rata, and a range of benefits, including opportunities for professional development and a supportive work environment.
