A talented, motivated, and dynamic Advanced Practitioner is needed to join our team. Your role as the Advanced Practitioner will be recognized as exemplary by your colleagues and other professionals in situations of high complexity. You will interact directly with the people who use services and their caregivers, as well as their families and communities. You will provide constructive feedback on practice, procedures, and policies, promote innovation, and implement new ways of working from established sites of excellence. Using critical reasoning and evidence-informed practice, you will develop knowledge and promote excellence in your field.

Our team is looking for someone who can demonstrate a strong belief in the concept of human rights in social work. You will be responsible for conducting complex assessments to promote the wellbeing of adults, including safeguarding their human rights. Understanding Article 5 and Article 8 ECHR rights, recognizing risks and preparing for emotional resilience within the context of the Human Rights Act, reflecting on and making informed judgments, and acting within local procedures, arrangements, and other roles. In particular, where a greater volume of experience and expertise is required.

The ability to communicate effectively, share appropriate, succinct, objective information and analysis to aid in joint decision-making with our health colleagues. Being proactive, persistent, and willing to challenge and be challenged. Effectively applying joint procedures and knowing your responsibilities and those of others.

The successful candidate will:

Have extensive experience in relevant settings after completing a degree in Social Work. Hold a DipSW or equivalent and be registered with Social Work England. CQSW/CSS/DIPSW BA/MA in Social Work Have proven experience of supervision or leadership skills. Have good working knowledge of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care. Be willing to deliver CHC training. Be willing to participate in CHC dispute resolution panels. You will have a minimum of one of the following qualifications: Best Interest Assessor (BIA) or Approved Mental Health Professional (AMHP) or Practice Educator full award. Have knowledge of relevant health and social care legislation. Hold a full UK Driving licence unless a disability prevents this.
