Job summary

We are a group of 3 GP Practices in the West of sheffield that are part of a larger Network,

We are looking for a Social Prescribing Link Worker to work with our adolescent population.

Social prescribing empowers people to take control of their own health and wellbeing through referrals to link workers who give time, focus on what matters to me and take a holistic approach to an individuals health and wellbeing. Link Workers also connect people to community groups and statutory services for practical and emotional support.

Main duties of the job

1. Take referrals from GP practices within Primary Care Network, providing practice and community-based consultations with patients.

2. Provide personalised support to individuals, to take control of their wellbeing.

3. Produce a personalised support plan to improve health and wellbeing, introducing or connecting people to community groups and statutory services.

4. Integrate into and form part of General Practice/Primary Care Network teams to provide the support needed in those communities they serve.

5. Liaise and communicate with Patients, Carers, Advocates, Health and Social Care professionals, voluntary sector and stake-holders involved in the wellbeing of your caseload and communities.

The role will require managing and prioritising your own caseload, It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals, when what the person needs is beyond the scope of the link worker role.

About us

We are 3 GP Practices in West Sheffield, Broomhill and Lodge Moor, Crookes and Selborne Road who are part of a larger network, West 5 PCN.

We currently have social prescribers working with our older population but we have a significant younger population who we feel w0uld benefit from this service

Job responsibilities

Promoting social prescribing, its role in self-management, and the wider determinants of health.

Build relationships with staff within the three GP practices within the three GP practices, attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing.

Be proactive in developing strong links with all local agencies to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.

Provide practices with regular updates about social prescribing, including training for their staff and how to access information to encourage appropriate referrals.

Seek regular feedback about the quality of service and the impact of social prescribing.

Meet people on a one-to-one basis by undertaking practice-based appointments or other appropriate venues

Give people time to tell their stories and focus on what matters to me. Build trust with the person, providing non-judgemental support and respecting diversity and lifestyle choices.

Be a friendly source of information about wellbeing and prevention approaches.

Help people identify the wider issues that impact on their health and wellbeing.

Work with individuals to co-produce a simple personalised action plan based on the persons priorities, interests, values and motivations.

Qualifications

Essential

It is essential that you have experience within a primary care setting or experience of a community, voluntary or social care setting. You will need to be a self-motivated, dedicated people-person whose passion is to support and empower individuals to take control of their health and wellbeing. IT skills

Ideally you will have worked in a role which has involved working 1:1 with patients/clients
