Main duties of the job

Wehave a great opportunity for a Band 6 Respiratory Physiotherapist to join ourteam delivering PR at local venues across East and South East Staffordshiredelivering high quality Pulmonary Rehabilitation (PR).

Aswell as seeing patients face to face - onsite in community-based venues, youwill have the opportunity to test your skills to expand the access for peoplerequiring PR.

Aspart of your role you will be responsible in supporting, organising and leadingthe team with group and 1:1 exercise and educational sessions for patients. Youwill complete in-depth assessments and build exercise plans to optimise thepatients physical capacity within the limitations of their respiratorydiagnosis, provide self-management plans and facilitate behavioural change forpatients to better manage their respiratory conditions and enhance theirquality of life.

Job responsibilities

Any reasonable duties to be undertaken to achieve the following:

To be responsible for providing respiratory physiotherapy interventions in community based Pulmonary Rehabilitation (PR) Programmes In conjunction with the Clinical Lead, support and develop both the onsite and digital delivery of the evidence based practice Pulmonary Rehabilitation programme. In conjunction with the Clinical Lead, provide supervision of junior staff members In collaboration with the Clinical Lead, support Physiotherapy student university placements To be actively involved in research and audit to ensure an evidence-based approach is used in physiotherapy practice to make informed judgements regarding patients care and service development. Ensuring CSP standards of practice, education and conduct are adhered to.

Principal Responsibilities

To assess, plan, coordinate and deliver all aspects of Pulmonary Rehabilitation (PR) across all geographical locations in South Staffordshire and South East Staffordshire, with support from the Clinical Team Deliver face to face pulmonary rehabilitation assessments and treatment programmes from the different sites in South and South East Staffordshire Deliver digital pulmonary rehabilitation using our CliniTouch Vie Platform, use video calls for assessment and appropriately prescribe treatment programmes Complete risk assessments and be innovative in the implementation of home-based treatment plans Assess Clinical outcome measures on both pre and post programme assessments and report on Minimal Clinical Improvement Difference (MCID) in post programme communication to GP Facilitate patients in completing their Patient Reported Outcome Measures (PROMs) both at pre-and post assessment Capture patient evaluations and feedback Audit and evaluate programme delivery and effectiveness of all programme types Deliver autonomous evidence based patient care Effective communication with admin, clinical and technical teams To be able to communicate complex and sensitive information to patients, carers and other staff relating to their rehabilitation potential. To use motivational skills to encourage patients to self-manage their conditions. To utilise experience and clinical knowledge to enable effective communication when dealing with complex/difficult patient situations where there may be conflict and a lack of acceptance of their respiratory condition In relation to PR provide patients with advice and treatment, any recommendations with patients specific respiratory and education needs and initiatives to improve health outcomes within your scope of practice Promote health through empowering patients, offering appropriate health education, information and advice. Refreshing Healthcare Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedure. To involve, support and educate patients and, as appropriate, their family and/or carers. To treat all patients as individuals, respecting their privacy and dignity at all times. To have an awareness of current national standards and guidelines relevant to Pulmonary Rehabilitation, Respiratory Disease and improve practice and health outcomes where appropriate. Participate in CPD (continuing professional development) opportunities to keep up to date with evidence-based knowledge and competence in all aspects of their role, meeting clinical governance guidelines for CPD. To maintain the high standards of the service by contributing towards individual and service outcomes. Delivering a quality service personal and people development and thus meeting the CQC standards. Recognise and work within own competence and maintain own standards and clinical integrity in line with Professional Body guidelines Partake in clinical supervision for own development and maintain a record of own personal development. To understand and value the roles of other members of the integrated service and to liaise and work with them as appropriat Promote effective communication and relationships within the team. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Evaluate patients response to the PR programme and the effectiveness of the service. Support and participate in shared learning across the clinical team. Undergo all mandatory training in line with role requirements. To use clinical reasoning skills when understanding the medical diagnosis/presentation and using knowledge of treatment options to plan, implement and adjust in a timely manner the individual PR programme. To provide specialist advice and support to patients to improve self- management of their condition. Utilising excellent verbal and non-verbal communication skills with patients, relatives, carers and members of the MDT to ensure that a high standard of patient centred care is achieved and that patients and carers have a good understanding of their individual plan. To determine the patients understanding of the programme in order to gain valid, informed consent and to be able to work within a legal framework with patients who lack capacity to consent to treatment. To assess and manage clinical risk. Collate, analyse and present clinical data and information to the team. Engages in annual appraisal, developing objectives to inform a Personal Development Plan. Participate in teaching where appropriate. Understands and demonstrates the characteristics of a role model to members in the team and/or service. Participates in investigation of incidents and events as required. Contributes to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Specific Skills/Competencies

Diploma or Degree Physiotherapy leading to inclusion on the HCPC Register. Evidence of post registration experience. Experience of working with patients with respiratory disease or other long-term conditions. Experience in delivering pulmonary rehabilitation would be desirable IT and data entry computer skills Awareness of current issues in NHS, including clinical governance. Knowledge of national guidelines pertinent to Respiratory conditions, including COPD. Knowledge of best practice in relation to respiratory conditions and pulmonary rehabilitation Excellent assessment skills Excellent clinical reasoning skills Team leadership skills Good communication skills Able to utilise current evidence base into practice. Essential

As a successful candidate, you will use your clinical expertise and decision-making skills to support patients throughout their pulmonary rehabilitation journey and to optimise the exercise and activity prescription leading to better patient outcome measures and ultimately an enhanced quality of life for them. We are looking for a qualified physiotherapist, with excellent communication and interpersonal skills, who is confident and experienced in managing patients with respiratory conditions. You will need to be able to work autonomously and support and lead the team. Experience of working in the community is desirable and you will need a full driving licence and access to a vehicle to travel to venues. All training and support will be given to the successful candidate.
