Job summary

This is an exciting opportunity for a newly created post to provide clinical leadership to the Community Parkinson's Practitioner Service across the whole of BNSSG. The post holder will be key in the continued development of this role and service and act as a fundamental resource for the team and work closely on the integration with our system partners in health, social care and VCSE sector.

The Specialist Parkinson's Practitioner service provides high-quality specialist advice and support for individuals with Parkinson's disease.

The service focuses on enabling individuals with Parkinson's to manage their condition independently, aiming to prevent complications and improve their quality of life.

The service ensures seamless care by fostering strong communication and partnership working between different sectors, including acute, community, primary care, and voluntary services.

This service is designed to provide comprehensive, specialised care to individuals with Parkinson's, ensuring they receive consistent support across all care settings whilst promoting self-management and reducing the burden on acute healthcare services.

Main duties of the job

- The Parkinson's Practitioners are responsible for assessing, planning, implementing, and monitoring care for people with Parkinson's. This includes developing person-centred care plans that are based on best practices and evidence.

- The practitioners provide specialist advice and support not only to patients and their families but also to other healthcare professionals involved in their care.

- Practitioners play a lead role in monitoring and developing the Parkinson's service to ensure it remains effective, safe, and sustainable. Working to ensure that care provided is of high quality and continuously improved.

- Providing psychological and emotional support to patients and their families is a critical component, especially during high-stress situations or when managing changes in the patient's condition.

- The service aims to reduce the need for acute care and emergency admissions by providing appropriate support and monitoring in the community, thereby facilitating early hospital discharge when necessary.

- The service also involves contributing to the education and development of other healthcare staff, ensuring they are well-equipped to provide care to Parkinson's patients.

- Practitioners contribute to clinical governance and are involved in audit and research activities, including supporting clinical trials and ensuring that the latest evidence-based practices are applied.

Skills and Experience

Essential

Demonstrable experience of working at Advanced Practice level within your clinical area Experience of leading care planning Experience of professional and management procedures within own area and across several related disciplines relevant to service area Demonstrates ability to take overall responsibility and accountability Experience of medicines management within your own practice either through an independent prescribing qualification or, if your professional registrations prohibits prescribing, through use of patient group directions or patient specific directives

Desirable

Experience of working at Advanced Practitioner level within a community health care setting Operational management experience including experience of managing and developing multi-disciplinary or integrated pathway teams in a variety of locations Experienced in application of change management principles

Qualifications

Essential

Registered Health Care professional NMC/HCPC or equivalent Completed the Advanced Practice MSc and been awarded the Centre for Advancing Practice (CfAP) Advanced Practice digital badge, OR been awarded another relevant MSc AND enrolled on the CfAP Supported (e-portfolio) route to gain your digital badge, OR you have demonstrable experience of working to an Advanced Practice level (MSc equivalent) across all four pillars AND you are enrolled on the CfAP Supported (e-portfolio) route to gain your digital badge

Desirable

Recognised post graduate management / leadership qualification, working towards one or willingness to undertake. Teaching qualification
